马应龙麝香痔疮膏对肛周脓肿术后患者症状控制、肛门功能及Col-Ⅰ、TGF-β1、VEGF、EGF水平的影响

摘要
目的 探讨马应龙麝香痔疮膏对肛周脓肿术后患者症状控制、肛门功能及胶原蛋白-Ⅰ(Col-Ⅰ)、转化生长因子-β1(TGF-β1)、血管内皮生长因子(VEGF)、表皮生长因子(EGF)水平的影响.方法 选择 2020 年 4 月—2023 年4 月收治的122 例肛周脓肿术后患者,按随机数字表法分为对照组和观察组各61 例.对照组给予生理盐水清创,观察组在对照组基础上联合马应龙麝香痔疮膏治疗.2 组均治疗3 周,随访6 个月.统计2 组术后3 周临床疗效及治疗期间不良反应发生情况,比较2 组术后1d和术后3 周症状控制情况、创面组织细胞因子表达情况、血清炎性因子水平及术前、术后6 个月肛门功能.结果 术后 3 周,观察组总有效率[91.80%(56/61)]高于对照组[78.69%(48/61)](P＜0.05).观察组术后3 周创面水肿、创面分泌物、肉芽组织及疼痛评分,血清白细胞介素(IL)-6、IL-8、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)水平及术后6 个月直肠静息压、肛管静息压、肛管最大收缩压、Wexner肛门失禁评分低于对照组,创面组织Col-Ⅰ、TGF-β1、VEGF、EGF表达水平高于对照组(P＜0.01).观察组治疗期间总不良反应发生率[4.92%(3/61)]与对照组[1.64%(1/61)]比较差异无统计学意义(P＞0.05).结论 肛周脓肿术后应用马应龙麝香痔疮膏治疗可有效改善创面组织细胞因子表达水平,减轻机体炎症反应,促进创面愈合,有助于患者症状控制,促进肛门功能转归,具有较好的治疗效果,同时安全性良好.
Abstract
Objective To investigate the effects of Mayinglong musk hemorrhoidal ointment on symptom control,anal function and levels of collagen-Ⅰ(COL-Ⅰ),transforming growth factor-β1(TGF-β1),vascular endothelial growth factor(VEGF)and epidermal growth factor(EGF)in patients with perianal abscess after operation.Methods A total of 122 pa-tients undergoing operation for perianal abscess treated from April 2020 to April 2023 were selected and divided into control group(n=61)and observation group(n=61)according to random number table method.The control group was given nor-mal saline debridement,and the observation group was treated with Mayinglong musk hemorrhoidal ointment on the basis of control group.Both groups were treated for 3 weeks and followed up for 6 months.Clinical efficacy at 3 weeks after operation and the occurrence of adverse reactions during treatment were analyzed in the two groups.Symptom control,wound cytokine expression,serum inflammatory factor level and anal function before and at 6 months after operation were compared between the two groups at 1 d and 3 weeks after surgery.Results At 3 weeks after operation,the total effective rate of the observation group[91.80%(56/61)]was higher than that of the control group[78.69%(48/61)](P＜0.05).Wound edema,wound secretion,granulation tissue and pain scores,serum leukocyte interleukin-6(IL-6),interleukin-8(IL-8),tumor nec-rosis factor-α(TNF-α),C reactive protein(CRP)levels at 3 weeks after operation,rectal resting pressure(RRP),resting pressure of anal canal,maximum systolic pressure(MSP)of anal canal and Wexner anal incontinence score at 6 months after operation in the observation group were lower than those in the control group,while the expressions of Col-Ⅰ,TGF-β1,VEGF and EGF in wound tissue were higher than those in control group(P＜0.01).There was no significant difference in the total incidence of adverse reactions between the observation group and the control group[4.92%(3/61)vs 1.64%(1/61)](P＞0.05).Conclusion After the operation for perianal abscess,Mayinglong musk hemorrhoidal ointment can effectively improve the expression level of wound cytokines,reduce the inflammatory response of the body,promote wound healing,help patients to control symptoms,and promote the outcome of anal function,with good therapeutic effect and good safety.
