摘要
目前鉴定壁外血管侵犯(EMVI)仍是直肠癌诊断和治疗的重大挑战,临床上通常利用直肠壁外血管形态学差异性来评估是否存在EMVI,但这种方法存在不确定性.MRI是术前预测EMVI的主要影像学检查方法,而常规MRI扫描、功能MRI定量分析和影像组学可以全面了解EMVI的本质和形态学相关信息,能够有效提高诊断EMVI的效能,有助于更好地协助临床诊断和治疗策略制订,可为影像学研究的临床转化铺平道路.本文综述了应用常规MRI、功能MRI及影像组学的定性和定量参数评估EMVI的研究进展.
Abstract
At present,the identification of extra-mural vascular invasion(EMVI)is still a major challenge in the di-agnosis and treatment of rectal cancer.The presence of EMVI is usually assessed by the morphological differences of extra-mu-ral vessels in the rectum,but this method is uncertain.Currently,MRI is the main imaging method for preoperative prediction of EMVI.Conventional MRI scanning,quantitative analysis of functional MRI and radiomics can comprehensively understand the essence and morphologic information of EMVI,effectively improve the efficiency of diagnosis of EMVI,and help to better assist clinical diagnosis and treatment strategy formulation,thus paving the way for the clinical transformation of imaging stud-ies.This paper reviews the progress of the evaluation of EMVI using qualitative and quantitative parameters of conventional MRI,functional MRI and radiomics.
