Objective To investigate the clinical manifestations,imaging features,diagnosis and treatment methods,causes of misdiagnosis and preventive measures of spinal dural arteriovenous fistula(SDAVF).Methods A retrospective analysis was conducted on the clinical data of two patients with SDAVF misdiagnosed as inflammatory myelopathy admitted from March 2016 to March 2018.Results Two patients were admitted to hospital due to progressive lower limb numbness,weakness with urine and stool disturbance for more than 5 months,and walking instability for 50 d,which was aggravated with neck pain and vomiting for 45 d,respectively.Both patients were misdiagnosed as inflammatory myelopathy,and the duration of misdiagno-sis was more than 5 months and 50 d,respectively.After careful examination of the film,it was found that the empty signal shadow of perimedullary flow could be seen on the magnetic resonance image,and the diagnosis of SDAVF was confirmed after performing the myelography.One patient died outside the hospital while waiting for interventional treatment in another hospital,and one died outside the hospital after routine interventional treatment.Conclusion The clinical manifestations of SDAVF are not typical,and it is more likely to be misdiagnosed in clinical setting.Careful inquiry of medical history,physical examination,careful reading imaging film,and spinal angiography when necessary are the key to avoid misdiagnosis of this disease.
