微通道植骨联合外固定架治疗桡骨远端C3型骨折的效果分析

摘要
目的 分析微通道植骨联合外固定架治疗桡骨远端C3 型骨折的临床效果.方法 选取 2021 年 6 月至2023 年6 月收治的59 例桡骨远端C3 型骨折患者,将应用微通道植骨联合外固定架治疗的30 例作为观察组,将应用闭合复位外固定架治疗的29 例作为对照组,观察比较2 组术后1 个月、术后3 个月、术后6 个月桡骨茎突高度、掌倾角和尺偏角;术后腕功能恢复情况,住院时间、术后6 个月骨折临床愈合率和术后随访情况.结果 术后1 个月、术后3 个月、术后6 个月2 组桡骨茎突高度、掌倾角及尺偏角比较差异无统计学意义(P＞0.05).观察组术后腕关节Cooney功能优秀率为70.0%高于对照组的37.9%(P＜0.05).2 组住院时间、术后6 个月骨折临床愈合率比较差异均无统计学意义(P＞0.05).2 组均无切口及钉道感染,骨折均愈合良好.观察组术后 1 个月复查X线显示桡骨远端断端对位对线良好.结论 应用微通道植骨联合外固定架治疗桡骨远端C3 型骨折,复位效果良好,桡骨关节面恢复平整,腕关节功能恢复良好.
Abstract
Objective To analyze the clinical effect of microchannel bone grafting combined with external fixation in the treatment of C3 fractures of distal radius.Methods In total,59 patients with type C3 fracture of distal radius treated from June 2021 to June 2023 were selected.Thirty patients treated with microchannel bone grafting combined with external fixation were selected as the observation group,and 29 patients treated with closed reduction and external fixation were selected as the control group.The height of radical styloid process,palmar inclination angle and ulnar deviation angle of the distal radius were observed and compared at 1 month,3 months and 6 months after surgery.Postoperative recovery of wrist function,length of hospital stay,clinical healing rate of fracture at 6 months after surgery and postoperative follow-up were compared between the two groups.Results There was no significant difference in the height of radical styloid process,palmar inclination angle and ulnar deviation angle of the distal radius between the two groups at 1 month,3 months and 6 months after surgery(P＞0.05).The excellent and good rate of Cooney wrist function score was 70.0%in the observation group,which was higher than that(37.9%)in the control group(P＜0.05).There were no significant differences in the length of hospital stay and the clinical healing rate of fractures at 6 months after surgery between the two groups(P＞0.05).There were no incision and nail tract infection in both groups,and the fractures healed well.In the observation group,X-ray reexamination at 1 month after opera-tion showed that the alignment of the broken end of the distal radius was good.Conclusion The application of microchannel bone grafting combined with external fixator in the treatment of type C3 fracture of distal radius has good reduction effect,smooth recovery of radial articular surface and good recovery of wrist joint function.
