Objective To investigate the clinical effect of detoxifying herbal lotion irrigation combined with vacuum sealing drainage(VSD)in the treatment of diabetic foot infection and its impact on inflammatory factors. Methods A total of 72 patients with diabetic foot infection treated from September 2023 to March 2024 were divided into control group and obser-vation group by random number table method, with 36 cases in each group.Basic treatment was performed in both groups.Of them,34 patients in the control group(1 patient with shedding,1 patient with uncertain efficacy)were treated with saline drip irrigation combined with VSD,and 33 patients in the observation group(1 patient dropping out of the study,2 patients with shedding)were treated with detoxifying herbal lotion irrigation combined with VSD.The clinical efficacy, serum inflam-matory factor levels and positive rate of bacterial culture in wound secretions at 1,2 weeks and 1 month after treatment were compared between the two groups, and adverse reactions were observed. Results The total effective rate was 90.91%(30/33)in the observation group, which was higher than that[67.65%(23/34)]in the control group(P<0.05).The time of wound cleansing, wound healing and total length of hospital stay in the observation group were shorter than those in the control group(P<0.05).At 1 and 2 weeks after treatment, the leukocyte count, neutrophil percentage,C reactive protein(CRP)and interleukin-6(IL-6)levels in the two groups were significantly decreased compared with those before treatment, which were lower in the observation group than in the control group(P<0.05).At 1 month after treatment, the leukocyte count, neutrophil percentage,CRP and IL-6 levels in the two groups were significantly decreased compared with those before treat-ment(P<0.05),but there was no statistical significance between the two groups(P>0.05).At1 and2 weeks after treat-ment,the positive rate of bacterial culture in wound secretions in observation group was lower than that in control group(P<0.05).There were no abnormalities in urine and stool routine and liver and kidney function in both groups. The incidence of adverse reactions was 3.03%(1/33)in the observation group and 5.88%(2/34)in the control group, with no significant significance(P>0.05).Conclusion Detoxifying herbal lotion irrigation combined with VSD in the treatment of diabetic foot infection can reduce the level of inflammatory factors, accelerate the elimination of pathogenic bacteria, promote wound recov-ery,and have good therapeutic effect and safety.
