参芪糖肾方对气阴两虚证糖尿病肾病患者临床疗效及血清肾纤维化标志物的影响

摘要
目的 探讨气阴两虚证糖尿病肾病(DKD)患者应用参芪糖肾方的疗效.方法 选取 2021 年 6 月至2024 年6 月收治的气阴两虚证DKD患者58 例,按照随机数字表法分为观察组和对照组,每组29 例,观察组采用常规治疗联合参芪糖肾方治疗,对照组采用常规治疗,比较2 组临床疗效、糖代谢指标、肾功能指标、血清肾纤维化标志物.结果 观察组总有效率高于对照组(P＜0.05).治疗后,观察组空腹血糖、餐后2h血糖、糖化血红蛋白均低于对照组(P＜0.05).治疗后,观察组估算肾小球滤过率高于对照组,尿白蛋白与肌酐比值低于对照组(P＜0.05).治疗后,观察组血清转化生长因子-β1、结缔组织生长因子均低于对照组(P＜0.05).2 组不良反应总发生率比较差异无统计学意义(P＞0.05).结论 气阴两虚证DKD患者联合应用参芪糖肾方治疗能够改善糖代谢及肾功能,在改善血清肾纤维化标志物及肾微血管病变方面也有优势,且安全性良好,临床疗效显著.
Abstract
Objective To investigate the effect of Shenqi Tangshen prescription on diabetic kidney disease(DKD)with Qi-Yin deficiency syndrome.Methods Fifty-eight DKD patients with Qi-Yin deficiency syndrome treated from June 2021 to June 2024 were selected and divided into observation group(n=29)and control group(n=29)according to random number table method.The observation group was treated with conventional treatment combined with Shenqi Tangshen prescrip-tion,and the control group was treated with conventional treatment.Clinical efficacy,glucose metabolism indexes,renal func-tion indexes and serum renal fibrosis markers were compared between the two groups.Results The total effective rate of ob-servation group was higher than that of control group(P＜0.05).After treatment,fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin in observation group were lower than those in control group(P＜0.05).After treatment,the estimated glomerular filtration rate of observation group was higher than that of control group,whereas the urina-ry albumin-to-creatinine ratio was lower than that of control group(P＜0.05).After treatment,the serum transforming growth factor β1 and connective tissue growth factor in the observation group were lower than those in the control group(P＜0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P＞0.05).Conclusion The combination of Shenqi Tangshen prescription for DKD patients with Qi-Yin deficiency syndrome can improve glucose metabolism and renal function,and has advantages in improving serum renal fibrosis markers and renal microvascular lesions,with good safety and significant clinical efficacy.
