目的 探讨肝癌组织中miR-92 与miR-4746-5p的表达水平及对预后的交互影响.方法 选取2021 年6 月至2023 年6 月收治的肝癌患者117 例作为恶性组,另选取同期肝脏良性肿瘤患者117 例作为良性组,对比2 组病灶组织及恶性组不同病理特征患者病灶组织miR-92、miR-4746-5p水平;对恶性组随访 1 年,根据预后情况分为预后良好亚组与预后不良亚组,比较2 亚组病灶组织miR-92、miR-4746-5p水平;分析miR-92、miR-4746-5p对肝癌预后的预测效能及交互影响.结果 恶性组病灶组织中miR-92、miR-4746-5p水平高于良性组(P<0.01);恶性组临床分期Ⅲ~Ⅳ期、有淋巴结转移、低分化、有门脉癌栓患者病灶组织miR-92、miR-4746-5p水平分别高于临床分期Ⅰ~Ⅱ期、无淋巴结转移、中高分化、无门脉癌栓患者(P<0.01).预后不良亚组有淋巴结转移、低分化病灶比例、有门脉癌栓比例及病灶组织 miR-92、miR-4746-5p 水平高于预后良好亚组(P<0.01);Spearman 相关性分析显示,miR-92、miR-4746-5p与肝癌临床分期、淋巴结转移、门脉癌栓呈正相关,与分化程度呈负相关(P<0.01).miR-92 与miR-4746-5p同时高表达提示肝癌预后不良风险增加16.000 倍,此时γ=1.287,表明miR-92 与miR-4746-5p高表达在肝癌预后中呈正向交互作用.结论 miR-92、miR-4746-5p在肝癌组织中高表达,与临床分期、淋巴结转移、门脉癌栓、分化程度临床病理特征显著相关,且二者同时高表达在预测预后中存在正向交互影响,能有效预测患者预后.
Expression Levels of miR-92 and miR-4746-5p in Liver Cancer Tissues and Their Interaction with Prognosis
Objective To investigate the expression levels of miR-92 and miR-4746-5p in liver cancer tissues and their interaction with prognosis.Methods A total of 117 patients with liver cancer admitted from June 2021 to June 2023 were selected as the malignant group,and another 117 patients with benign liver tumor during the same period were selected as the benign group.The levels of miR-92 and Mir-4476-5p in lesion tissues of the two groups and patients with different patho-logical characteristics of the malignant group were compared.The malignant group was followed up for 1 year and divided into good prognosis subgroup and poor prognosis subgroup according to the prognosis.The levels of miR-92 and miR-4746-5p in le-sion tissues of the two subgroups were compared.The predictive efficacy and interaction effects of miR-92 and miR-4746-5p on prognosis were analyzed.Results The levels of miR-92 and miR-4746-5p in lesions tissues of malignant group were higher than those in benign group(P<0.01).In the malignant group,the levels of miR-92 and miR-4746-5p in lesion tissue of pa-tients with clinical stage Ⅲ to Ⅳ,lymph node metastasis,low differentiation,and portal vein cancer thrombus were higher than those of patients with clinical stage Ⅰ to Ⅱ,no lymph node metastasis,medium and high differentiation,and portal vein cancer thrombus,respectively(P<0.01).The proportion of lymph node metastasis,poorly differentiated lesions,the propor-tion of lesions with portal vein cancer embolus and the levels of miR-92 and miR-4746-5p in lesion tissue in the poor prognosis group were higher than those in the good prognosis group(P<0.05).Spearman correlation analysis showed that miR-92 and miR-4746-5p were positively correlated with clinical stage,lymph node metastasis and portal vein cancer thrombus,but nega-tively correlated with differentiation degree of liver cancer(P<0.01).The simultaneous high expression of miR-92 and miR-4746-5p suggested a 16.000 fold increase in the risk of poor prognosis of liver cancer,and γ=1.287,indicating a positive in-teraction between the high expression of miR-92 and miR-4746-5p in the prognosis of liver cancer(P<0.05).Conclusion The high expression of miR-92 and miR-4476-5p in liver cancer tissues is significantly correlated with clinicopathological fea-tures,and the high expression of both has a positive interaction in predicting prognosis,which can effectively predict the prog-nosis of patients.
