Association between visceral fat index and the risk of cardiovascular disease in middle-aged and elderly people in China

摘要
目的:探究中国人内脏脂肪指数(Chinese visceral adiposity index,CVAI)与中老年心血管疾病发病风险的关联.方法:以2011年中国健康与养老追踪调查(China Health and Retirement Longitudinal Study,CHARLS)的45岁及以上的中老年人信息作为基线数据,并随访至2018年.采用Cox比例风险模型分析不同CVAI水平与心血管疾病发病的关联.结果:研究纳入6 484例基线时无心血管疾病的研究对象,平均年龄为(57.8±9.0)岁.在长达7年的随访期间,心血管疾病的发生率为7.0%.Cox比例风险模型结果表明,在调整潜在的混杂因素后,CVAI每增加1个标准差水平,心血管疾病发病风险增加15%(HR=1.15,95%CI:1.04~1.30),二者存在非线性正相关.亚组分析结果显示,在年龄>65岁、男性、合并高血压、血脂异常、糖尿病和卒中的人群中,CVAI与心血管疾病发病风险关联不显著.结论:CVAI与中老年心血管疾病发病相关联,在相对年轻的中老年人群的心血管病一级预防中具有应用潜力.
Abstract
Objective:To explore the association between Chinese visceral adiposity index(CVAI)and incident cardiovascular diseases among the Chinese older population.Methods:The baseline data were derived from partici-pants aged 45 years and older in the China Health and Retirement Longitudinal Study(CHARLS)dataset in 2011.Participants were followed up until 2018.Cox proportional models were applied to investigate the association be-tween CVAI and the incidence of cardiovascular diseases.Results:A total of 6 484 participants without prevalent cardiovascular diseases at baseline were included,with a mean age of(57.8±9.0)years.During the 7-ear follow-up period,the incident rate of cardiovascular diseases was 7.0%.After adjusting for potential confounders,the Cox model showed that the risk of cardiovascular diseases increased by 15%(HR=1.15,95%CI:1.04-1.30)per standard deviation increase in CVAI.The association between CVAI and cardiovascular diseases exhibited a non-linear pattern.Subgroup analysis indicated that CVAI was not significantly associated with cardiovascular dis-eases among participants aged older than 65 years or those with hypertension,dyslipidemia,diabetes,and stroke at baseline.Conclusion:CVAI is significantly associated with the incidence of cardiovascular diseases among the Chinese older population,highlighting its potential role in the primary care of cardiovascular diseases,particularly among relatively younger populations.
