Relationship between miR-126 expression in bone marrow of elderly patients with acute myeloid leukemia and clinical characteristics and chemotherapy efficacy
Objective:To analyze the correlation between miR-126 expression in the bone marrow of elderly patients with acute myeloid leukemia(AML)and clinical characteristics as well as chemotherapy efficacy.Meth-ods:The clinical data of 85 elderly AML patients treated in our hospital from June 2019 to June 2022 were retro-spectively analyzed.The relative expression of miR-126 in bone marrow was detected before chemotherapy.The relative expression of miR-126 in bone marrow of the patients with different clinical characteristics was compared.The correlation between miR-126 expression in bone marrow of AML patients and clinical characteristics and the related factors affecting miR-126 expression in bone marrow were analyzed.After chemotherapy,the efficacy was evaluated and the patients were divided into ineffective group(36 cases)and effective group(49 cases).The receiv-er operating characteristic(ROC)curve was used to analyze the predictive value of miR-126 expression in bone marrow for the efficacy of chemotherapy in elderly AML patients.The optimal threshold was obtained by ROC curve.According to the optimal threshold,AML patients were further divided into miR-126 high expression group(35 cases)and low expression group(50 cases).The survival outcomes of the patients with high expression and low expression of miR-126 were compared.Results:Comparison of miR-126 relative expression levels in the bone marrow of the patients with different classification diagnostic criteria(FAB classification),prognostic risk stratification,and proportion of bone marrow primitive cells for acute leukemia,had a statistical significant differ-ence(P<0.05).Through Spearman correlation test,the results showed that the relative expression level of miR-126 in the bone marrow of AML patients was related to FAB typing,prognostic risk stratification,and the pro-portion of bone marrow primitive cells(P<0.05).Through linear regression analysis,the results showed that the influencing factors of miR-126 expression in the bone marrow of AML patients were FAB typing,prognostic risk stratification,and the proportion of bone marrow primitive cells(P<0.05).After one course of treatment,the total effective rate of treatment for 85 elderly AML patients was 57.65%.Before treatment,the relative expres-sion level of miR-126 in the bone marrow of the ineffective group was higher than that of the effective group,with a statistical significant difference(P<0.05).After treatment,the relative expression level of miR-126 in the bone marrow of both groups decreased compared to before treatment,with a statistical significant difference(P<0.05),while the relative expression level of miR-126 in the bone marrow of the two groups was not statistically significant(P>0.05).ROC curves were drawn,and the results showed that the area under the curve for miR-126 to predict chemotherapy efficacy in elderly AML patients was 0.824,with specificity and sensitivity of 77.30%and 78.90%,respectively.The ROC curve was used to intercept the optimal threshold(1.145)for predicting the chemotherapy efficacy of miR-126 in AML patients.Grouping was performed based on the optimal threshold,and the relative expression level of miR-126 in the bone marrow was defined as high expression(35 cases),while<1.145 was defined as low expression(50 cases).The Kaplan-Meier survival curve was plotted,and the results showed that the survival time of the miR-126 high expression group was significantly shorter than that of the miR-126 low expression group(P<0.05).Conclusion:The expression of miR-126 in the bone marrow of elderly AML patients may be related to clinical characteristics and chemotherapy efficacy,and the expression of miR-126 in the bone marrow before chemotherapy may have certain value in predicting the chemotherapy efficacy of patients.
