Characteristics of thromboelastogram parameters and analysis of clinical influencing factors during pregnancy
Objective:To study the differences of TEG parameters in different gestational periods and compare them with the normal population,analyze the effects of hypertensive disorders in pregnancy(HDP),gestational diabetes mellitus(GDM),age,mode of conception,gestational stage,platelet count and hemoglobin level on TEG parameters during pregnancy.To explore the correlation between TEG parameters and other clinical indicators.Methods:A total of 265 pregnant women who visited the department of obstetrics and gynecology of our hospital from January 2020 to April 2023 were retrospectively analyzed as the observation group,and 316 female healthy people who underwent physical examination in the physical examination department of our hospital during the same period were selected as the control group.The observation group was divided into 4 groups according to dis-ease type:healthy pregnancy group(191 cases),HDP group(26 cases),GDM group(38 cases),HDP combined with GDM group(10 cases).TEG,four items of coagulation,blood routine,and clinical indicators such as age,pregnancy,complications,and mode of conception were measured in the observation group,and TEG was meas-ured in the control group.Results:A compared with the normal population,the α angle,CI and MA values of the pregnant group increased,while the R and K values decreased(P<0.001).There were significant differences in the parameters of TEG in each trimester(P<0.05).Multiple linear regression analysis showed that the R value was affected by hypertension(P<0.05)and pregnancy stage(P<0.05),the K value was affected by age(P<0.05),pregnancy stage(P<0.001)and platelet count(P<0.05),and the α angle value was affected by pregnan-cy stage(P<0.001).The MA value was affected by the stage of pregnancy(P<0.001)and platelet count(P<0.001),and the CI value was affected by the stage of pregnancy(P<0.001)and platelet count(P<0.05),while the TEG parameters were not different among the groups of healthy pregnancy,GDM,HDP and GDM with HDP(P>0.05).The platelet count,hemoglobin level,PT,APTT,Fib are correlated with TEG parameters during pregnancy.Conclusion:TEG parameters are different in different pregnancy periods,and are related to some indi-cators of blood routine and coagulation function.TEG parameters are affected by hypertension,pregnancy stage,age,platelet count and other factors,which have unique value in the evaluation of coagulation function during pregnancy.
