Impact of anti-M antibody on transfusion compatibility test and transfusion strategy in tumor patients
Objective:To analyze the influence of anti-M antibody on blood transfusion compatibility test,and to discuss the clinical countermeasures in clinical blood transfusion.Methods:Specimens were collected from 21 cases with identified anti-M antibodies in our hospital between January 2023 and March 2024.The influence of an-ti-M antibody on ABOblood group identification was analyzed,and the specificity,type,and titer of the antibody were determined.Appropriate blood was screened for cross-matching in cases with detected anti-M antibodies.Results:Among the 21 cases,there were discrepancies in ABO blood group typing for 10 cases.In irregular anti-body screening at 37℃,20 cases tested positive while 1 cases were missed.The missed cases were detected during pre-matching stage.Anti-M antibodies were found to be IgM in 5 cases,IgG in 10 cases,and IgG+IgM in 6 ca-ses.In the titer test using the saline method,all cases showed activity at 4℃,16 cases at 24℃,and 10 cases at 37℃.Antibody screening using a room temperature saline method combined with antibody identification at 4℃can assist in identifying the antibodies.Two cases finally received red blood cell transfusion without adverse reac-tions and clinical conditions remained stable.Conclusion:Anti-M antibodies can lead to ABO typing incompatibili-ty.There is a possibility of missed detection during irregular antibody screening conducted at 37℃.The probabili-ty of missed detection can be reduced by various means such as antibody screening by room temperature saline method,adjustment of incubation temperature,and pre-matching strategy.For patients with anti-M antibodies,M-antigen negative blood should be selected for preparation and transfusion.
