南通地区RhD阳性献血者表型分布及输血策略研究

目的:分析南通地区RhD阳性献血者C、c、E、e抗原分布特点及表型规律,为制定Rh血型相容性输血策略提供依据.方法:随机选取2023年9月5日-2023年11月7日南通地区无偿献血者样本,用微柱凝胶卡式法对其进行C、c、E、e抗原检测以确定表型.采用x2检验与Fisher精确检验对各地区抗原频率分布情况、Rh血型表型等进行统计学分析.结果:1 534例南通本地RhD阳性献血者C、c、E、e抗原频率分布趋势为e(93.42%)>C(89.50%)>c(58.67%)>E(50.59%),表型分布趋势为 Ce(40.35%)>CcEe(38.98%)>Cce(8.80%)>cE(6.19%)>cEe(4.04%)>CEe(0.98%)>CcE(0.39%)>ce(0.26%)>CE(0),不同表型相容性概率由高到低为 CcEe(100.00%)>Cce(49.41%)>CEe(41.33%)>Ce(40.35%)>cEe(10.23%)>CcE(6.58%)>cE(6.19%)>ce(0.26%)>CE(0).男女间Rh血型分布频率差异无统计学意义(P>0.05).南通、南宁、广州、深圳及福州的Rh血型分布差异有统计学意义(x2=315.626,P<0.001).结论:南通地区RhD阳性献血者表型分布存在地域差异,有其自身特点.在开展Rh血型相容性输注时,应考虑地域性表型分布特征,选择适宜的相容性输血策略,实施精准输血以保障安全.
Phenotypic distribution and transfusion strategy of RhD-positive blood donors in Nantong
Objective:To analyze the profiles of RhC,c,E,and e antigens and phenotypes in RhD-positive blood donors from Nantong,so as to provide data for compatibility blood transfusion of Rh blood group.Methods:Blood donors in Nantong from September 5,2023 to November 7,2023 were randomly selected,and C,c,E,and e antigens were detected by microcolumn gel card method to determine the Rh phenotype.The Chi-sequare test and Fisher's exact test were used to analyze antigen frequency and Rh phenotypes in each region.Results:Among the 1 534 Rh-positive blood donors,the frequency of C,c,E,and e antigen was e(93.42%)>C(89.50%)>c(58.67%)>E(50.59%),and Rh phenotypes distribution was Ce(40.35%)>CcEe(38.98%)>Cce(8.80%)>cE(6.19%)>cEe(4.04%)>CEe(0.98%)>CcE(0.39%)>ce(0.26%)>CE(0),the probability of compatibili-ty between different phenotypes was CcEe(100.00%)>Cce(49.41%)>CEe(41.33%)>Ce(40.35%)>cEe(10.23%)>CcE(6.58%)>cE(6.19%)>ce(0.26%)>CE(0).Difference of Rh phenotype distribution between male and female were not noticed in Nantong(P>0.05).There was significant difference in Rh blood type distri-bution among Nantong,Nanning,Guangzhou,Shenzhen and Fuzhou(x2=315.626,P<0.001).Conclusion:The phenotypic distribution characteristics of RhD-positive blood donors in Nantong have regional differences.The re-gional phenotypic distribution characteristics should be considered to select an appropriate compatible transfusion strategy,and precise transfusion should be implemented to ensure safety when implementing Rh compatible trans-fusion.
