Improvement effect and safety evaluation of methylcobalamin combined with prednisone on vertigo degree and balance function in elderly patients with vestibular neuritis
Objective To evaluate the improvement effect and safety of methylcobalamin combined with prednisone on vertigo degree and balance function in elderly patients with vestibular neuritis.Methods A total of 100 elderly patients with vestibular neuritis admitted from April 2018 to April 2022 were selected as the research objects and divided into routine group(50 cases,prednisone treatment)and combined group(50 cases,methylcobalamin combined with prednisone treatment)by random red-blue ball method.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the combined group was significantly higher than that in the routine group(P<0.05).After treatment,the scores of Dizziness Handicap Inventory(DHI)and Vestibular Disorders Activities of Daily Living Scale(VADL)in the combined group were significantly lower than those in the routine group,and the Berg Balance Scale(BBS)score was significantly higher than that in the routine group(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interferon-γ(INF-γ),interleukin-17(IL-17)and interleukin-8(IL-8)in the combined group were significantly lower than those in the routine group(P<0.05).After treatment,the levels of 8-hydroxydeoxyguanosine(8-OHdG)and malondialdehyde(MDA)in the combined group were significantly lower than those in the routine group,and the levels of superoxide dismutase(SOD),total antioxidant capacity(T-AOC)and glutathione(GSH)were significantly higher than those in the routine group(P<0.05).After treatment,the levels of neurotrophic factor(NTF),5-hydroxytryptamine(5-HT)and 25-hydroxyvitamin D(25-OHD)in the combined group were significantly higher than those in the routine group,and the level of methionine enkephalin(MEK)was significantly lower than that in the routine group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Methylcobalamin combined with prednisone in the treatment of elderly patients with vestibular neuritis has a significant effect,it can quickly improve the symptoms of patients,eliminate inflammatory response and oxidative stress response,and improve neurotrophic indicators.
