目的 分析血栓通联合阿替普酶治疗急性脑梗死的效果及对脑血流动力学的影响。方法 选取2015 年4 月至 2021 年 10 月收治的 200 例急性脑梗死患者为研究对象,对其进行编号,依据编号的奇偶性将其分为对照组和观察组,各100例。对照组采用阿替普酶治疗,观察组在对照组基础上联合血栓通治疗。比较两组的治疗效果。结果 观察组的治疗总有效率高于对照组(P<0。05)。治疗后,观察组的全血高切黏度(HBV)、全血低切黏度(LBV)、血细胞比容(HCT)及血沉(ESR)低于对照组(P<0。05)。治疗后,观察组椎动脉(VA)和双侧大脑中动脉(MCA)的平均血流速度(Vm)及收缩期峰流速度(Vs)高于对照组,美国国立卫生研究院卒中量表(NIHSS)评分低于对照组(P<0。05)。治疗后,观察组的S-100β、内皮缩血管肽-1(ET-1)、可溶性细胞间黏附分子-1(sICAM-1)及神经元特异性烯醇化酶(NSE)水平低于对照组(P<0。05)。两组的不良反应总发生率无显著差异(P>0。05)。结论 血栓通联合阿替普酶治疗急性脑梗死的效果良好,可改善患者的脑血流动力学、血液流变学指标,提升神经功能,且安全性理想,具有重要应用价值。
Effect of Xueshuantong combined with alteplase in the treatment of acute cerebral infarction and its influence on cerebral hemodynamics
Objective To analyze the effect of Xueshuantong combined with alteplase in the treatment of acute cerebral infarction and its influence on cerebral hemodynamics.Methods A total of 200 patients with acute cerebral infarction admitted from April 2015 to October 2021 were selected as the research objects and numbered.According to the parity of the number,the patients were divided into control group and observation group,with 100 cases in each group.The control group was treated with alteplase,and the observation group was treated with Xueshuantong on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the whole blood high shear viscosity(HBV),whole blood low shear viscosity(LBV),hematocrit(HCT)and erythrocyte sedimentation rate(ESR)in the observation group were lower than those in the control group(P<0.05).After treatment,the mean velocity(Vm)and systolic velocity(Vs)of vertebral artery(VA)and middle cerebral artery(MCA)in the observation group were higher than those in the control group,and the National Institutes of Health Stroke Scale(NIHSS)score was lower than that in the control group(P<0.05).After treatment,the levels of S-100β,endothelin-1(ET-1),soluble intercellular adhesion molecule-1(sICAM-1)and neuron-specific enolase(NSE)in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Xueshuantong combined with alteplase in the treatment of acute cerebral infarction has a good effect,it can improve the cerebral hemodynamics and hemorheology indexes of patients,and improve neurological function,which has ideal safety and important application value.
