目的 分析骨质疏松性椎体压缩性骨折(OVCF)经皮椎体成形术(PVP)后骨水泥弥散分布情况的影响因素。方法 收集2020 年1 月至 2022 年 9月在广东医科大学附属医院行PVP的 172 例OVCF患者临床和影像学资料,根据术后DR正侧位片复查结果确定骨水泥弥散范围,并将其分为弥散不佳组和弥散优良组。统计两组的骨密度值、手术入路、黑线征、Kummell病、手术时机、骨髓水肿形态、骨折类型及骨水泥体积分数。分析骨水泥弥散分布情况的影响因素。结果 骨水泥弥散分布情况显示,弥散不佳100例,弥散优良72例。两组的骨密度值、手术入路、Kummell病占比、手术时机、骨髓水肿形态比较,差异具有统计学意义(P<0。05);两组的黑线征占比、骨折类型、骨水泥体积分数比较,差异无统计学意义(P>0。05)。多因素Logistic回归分析结果显示,手术入路、Kummell病、手术时机、骨髓水肿形态是骨水泥弥散分布情况的影响因素(P<0。05),其中手术入路为正相关因素,Kummell病、手术时机和骨髓水肿形态为负相关因素。结论 手术入路、Kummell病、手术时机、骨髓水肿形态是骨水泥弥散分布情况的影响因素。
Analysis of influencing factors of bone cement dispersion distribution after percutaneous vertebroplasty for osteoporotic vertebral compression fracture
Objective To analyze the influencing factors of bone cement disp ersion distribution after percutaneous vertebroplasty(PVP)for osteoporotic vertebral compression fracture(OVCF).Methods The clinical and imaging data of 172 patients with OVCF who underwent PVP in the Affiliated Hospital of Guangdong Medical University from January 2020 to September 2022 were collected,the diffusion range of bone cement was determined according to the results of postoperative DR positive and lateral radiographs,and the patients were divided into poor dispersion group and excellent dispersion group.The bone mineral density,surgical approach,black line sign,Kummell disease,surgical timing,bone marrow edema morphology,fracture type and bone cement volume fraction of the two groups were statistically analyzed.The influencing factors of bone cement dispersion distribution were analyzed.Results The dispersion distribution of bone cement showed that there were 100 cases of poor dispersion and 72 cases of excellent dispersion.There were statistically significant differences in bone mineral density,surgical approach,proportion of Kummell disease,surgical timing and bone marrow edema morphology between the two groups(P<0.05);there were no significant differences in the proportion of black line sign,fracture type and bone cement volume fraction between the two groups(P>0.05).Multivariate Logistic regression analysis showed that surgical approach,Kummell disease,surgical timing and bone marrow edema morphology were the influencing factors of bone cement dispersion distribution(P<0.05),among them,surgical approach was a positive correlation factor,Kummell disease,surgical timing and bone marrow edema morphology were negative correlation factors.Conclusion Surgical approach,Kummell disease,surgical timing and bone marrow edema morphology are influencing factors of bone cement dispersion distribution.
