Effects of symptom management strategy combined with trinity continuous nursing model on disease control and home self-management ability of patients with chronic heart failure
Objective To explore the effects of symptom management strategy combined with trinity continuous nursing model on disease control and home self-management ability of patients with chronic heart failure.Methods A total of 90 patients with chronic heart failure admitted from January 2020 to January 2022 were selected as the research objects,and the patients were divided into control group and observation group according to the odd and even number of beds,with 45 cases in each group.The control group was given routine nursing intervention,and the observation group was given symptom management strategy combined with trinity continuous nursing model on the basis of the control group.The intervention effects of the two groups were compared.Results After intervention,the scores of each dimension of Memorial Symptom Assessment Scale-Heart Failure(MSAS-HF)in the observation group were lower than those in the control group(P<0.05).After intervention,the scores of home medication,home diet,disease self-monitoring and home cardiac rehabilitation exercise in the observation group were higher than those in the control group(P<0.05).After intervention,the left ventricular ejection fractions(LVEF)and left ventricular time to peak ejection rate(LVTPER)in the observation group were higher than those in the control group,and the left ventricular peak ejection rate(LVPER)was lower than that in the control group(P<0.05).Conclusion The symptom management strategy combined with trinity continuous nursing model can not only increase the disease control of patients with chronic heart failure,but also improve the home self-management ability of patients,thus effectively improving cardiac function.
