Observation on the effect of one-stage combined anteroposterior approach in the treatment of complex Pilon fracture
Objective To observe the feasibility and effect of one-stage posterolateral approach combined with anterior median approach(one-time completion of operation)in the treatment of complex Pilon fracture.Methods A total of 9 patients with complex Pilon fracture admitted in orthopaedics department of emergency trauma center,Henan Province Hospital of TCM from September 2016 to November 2019 were retrospectively selected,all of them completed the operation at one time through the posterolateral approach combined with anterior median approach after proper preparation,and followed up to observe the therapeutic effect.Results Burwell-Charnley imaging evaluation was performed at 3 months after operation,and the results were excellent in 7 cases,good in 2 cases,and poor in 0 case.According to the American Orthopaedic Foot and Ankle Society(AOFAS)ankle-hindfoot score at the last follow-up,5 cases were excellent,3 cases were good,1 case was medium,and 0 case was poor.Fracture reduction and ankle function recovery were satisfactory.Conclusion With proper preparation and selection of appropriate cases,the scheme of one-stage combined anteroposterior approach can be selected for complex Pilon fracture,and has obvious advantages.
