目的 探讨 2 型糖尿病(T2DM)患者周围神经传导速度异常的影响因素。方法 选取 2021 年 1 月至2023年1月我院收治的156 例T2DM患者为研究对象,将合并周围神经病变(PN)的 85 例患者设为糖尿病周围神经病变(DPN)组,未合并PN的71 例患者设为NDPN组。两组均行肌电图检查,比较周围神经传导速度;比较两组的基础资料、生化指标;应用多因素Logistic回归分析DPN的影响因素。结果 DPN组的正中感觉神经传导速度、正中运动神经传导速度、双侧腓肠感觉神经传导速度、双侧腓总运动神经传导速度低于NDPN组(P<0。05)。两组的T2DM病程、餐后 2h血糖(2hPG)、空腹血糖(FPG)、餐后 2hC肽(2hC-P)、糖化血红蛋白(HbA1c)、尿微量白蛋白(UMA)、血浆渗透压(POP)水平比较,差异具有统计学意义(P<0。05)。多因素Logistic回归分析显示,T2DM病程、2 h PG、FPG、2 h C-P、HbA1c、UMA、POP是DPN的影响因素(P<0。05)。结论 DPN发病机制较为复杂,临床应监测T2DM患者病程、2 h PG、FPG、2 h C-P、HbA1c、UMA、POP,定期进行肌电图检查,尽可能预防及延缓DPN发生发展。
Influencing factors of abnormal peripheral nerve conduction velocity in patients with type 2 diabetes mellitus
Objective To investigate the influencing factors of abnormal peripheral nerve conduction velocity in patients with type 2 diabetes mellitus(T2DM).Methods A total of 156 patients with T2DM admitted in our hospital from January 2021 to January 2023 were selected as the research objects.Eighty-five patients with peripheral neuropathy(PN)were set as diabetic peripheral neuropathy(DPN)group,and 71 patients without PN were set as NDPN group.Both groups were examined by electromyography,and the peripheral nerve conduction velocity was compared;the basic data and biochemical indexes of the two groups were compared.Multivariate Logistic regression was used to analyze the influencing factors of DPN.Results The median sensory nerve conduction velocity,median motor nerve conduction velocity,bilateral sural sensory nerve conduction velocity and bilateral peroneal motor nerve conduction velocity in the DPN group were lower than those in the NDPN group(P<0.05).There were statistically significant difference in course of T2DM,2 h postprandial blood glucose(2 h PG),fasting plasma glucose(FPG),2 h connecting peptide(2 h C-P),glycated hemoglobin(HbA1c),urinary microalbumin(UMA)and plasma osmotic pressure(POP)between the two groups(P<0.05).The multivariate Logistic regression analysis showed that the course of T2DM,2 h PG,FPG,2 h C-P,HbA1c,UMA and POP were the influencing factors of DPN(P<0.05).Conclusion The pathogenesis of DPN is relatively complex,and clinical monitoring of the course of T2DM patients,2 h PG,FPG,2 h C-P,HbA1c,UMA and POP should be carried out regularly.Electromyography should be performed to prevent and delay the occurrence and development of DPN as much as possible.
