宏基因组二代测序在涂阴性肺结核中的应用进展

结核病是全球关注的公共卫生问题,全球结核病、耐药结核病以及结核病死亡患者数量都在逐步增长,我国结核病形势也不容乐观。早期准确的病原学诊断是有效治疗的关键,但我国大多数的结核患者病原学检测阴性或缺乏病原学检测结果。对于病原学阴性肺结核,目前常用病原学检测方法尚不能满足临床的诊断需求。宏基因组二代测序(mNGS)的出现颠覆了传统的结核杆菌检测技术以及病原学诊断方法,为涂阴性肺结核的早期诊断、早期治疗提供了可靠参考。本文就现有结核分枝杆菌(MTB)检测方法及国内外开展的mNGS技术的应用作一个综述,重点分析mNGS在涂阴性肺结核诊断中的优势与局限性,为临床有效诊断涂阴性肺结核及合理应用mNGS提供参考。
Application progress of metagenomic next-generation sequencing in smear negative pulmonary tuberculosis
Tuberculosis is a public health problem of global concern.The number of global tuberculosis,drug-resistant tuberculosis and tuberculosis deaths is gradually increasing,and the situation of tuberculosis in China is not optimistic.Early and accurate etiological diagnosis is the key to effective treatment,but most of the tuberculosis patients in China are negative or lack of etiological test results.For pathogenic negative pulmonary tuberculosis,the current commonly used pathogenic detection methods can not meet the clinical diagnostic needs.The emergence of metagenomic next-generation sequencing(mNGS)has subverted the traditional tuberculosis detection technology and etiological diagnosis method,and provided a reliable reference for the early diagnosis and early treatment of smear negative pulmonary tuberculosis.This article reviews the existing detection methods of Mycobacterium tuberculosis(MTB)and the application of mNGS technology at home and abroad,and focuses on the advantages and limitations of mNGS in the diagnosis of smearnegative pulmonary tuberculosis,so as to provide reference for clinical effective diagnosis of smear negative pulmonary tuberculosis and rational application of mNGS.
