A case report and literature review of a patient with hypoparathyroidism with epilepsy as the first manifestation
Hypoparathyroidism(HP)is a clinical syndrome characterized by hypocalcemia,hyperphosphatemia,soft tissue ectopic calcification,and increased neuromuscular excitability due to insufficient secretion and/or insufficie nt effect of parathyroid hormone(PTH).Recently,our hospital admitted a patient with recurrent epilepsy as a manifestation of HP,and the patient had recurrent episodic loss of consciousness,binocular gaze,closed teeth,and convulsion of the limbs more than 10 years ago.Each episode lasted for about a few minutes,and the above symptoms could improve on their own.After waking up,the patient could not recall the course of the disease.The patient were diagnosed with epilepsy at an external hospital and treated with regular antiepileptic drugs.During the treatment,the above symptoms still recurred,with a low initial frequency of onset,about once a year.In the past two years,the frequency of seizures has increased,with an average of once every 2 to 3 months.Three days before admission,there was a recurrence of consciousness loss,binocular gaze,closed teeth,and convulsion of the limbs due to catching a cold.The seizures occurred once and lasted for about 10 minutes before awakening on their own.After waking up,they experienced dizziness,limb weakness,accompanied by poor mental state,poor appetite,cough,sputum,and other clinical manifestations,and the patient was treated at our department.We summarize the incidence,diagnosis and treatment process of this case,and review relevant literature in order to deepen the understanding of HP related epilepsy among clinical physicians.
