Evaluation of left ventricular systolic function in patients with ventricular hypertrophy by tissue motion annular displacement
Objective To explore the application value of automatic tracking technique of tissue motion annular displacement(TMAD)in quantitative evaluation of left ventricular systolic function in patients with ventricular hypertrophy. Methods A total of 60 patients with ventricular hypertrophy were included, including 30 patients in primary hypertension subgroup and 30 patients in uremia subgroup, and 35 healthy subjects were included as control group. The two-dimensional speckle tracking technique was used to automatically obtain the TMAD values of the four sites of the mitral annulus in the apical four-chamber view and the apical two-chamber view, the left ventricular global longitudinal strain(LVGLS)and the left ventricular global circumferential strain(LVGCS).The Bland-Altman method was used to test the consistency of TMAD parameters and the receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic efficacy of TMAD parameters. Results The systolic blood pressure(SBP)and diastolic blood pressure(DBP)in the primary hypertension subgroup and the uremia subgroup were significantly higher than those in the control group(P<0.05).The heart rate(HR)of the uremia subgroup was higher than that of the primary hypertension subgroup and the control group(P<0.05).The TMAD parameters and strain parameters of the uremia subgroup were significantly lower than those of the control group(P<0.05).LVGLS,T-AP4sep,T-AP4mid%,T-AP2int,T-AP2ant and T-AP2mid%in the primary hypertension subgroup were lower than those in the control group(P<0.05).ROC curve results showed that TMAD parameters had moderate predictive value for left ventricular systolic function in patients with ventricular hypertrophy. Conclusion TMAD can quantitatively evaluate left ventricular systolic function in patients with ventricular hypertrophy, and has good repeatability.
