Effect of pre-prescription review on clinical rational application of esomeprazole
Objective To explore the effect of fine setting of audit rules by pre-prescription system on the clinical rational use of esomeprazole.Methods The drug use evaluation(DUE)standard of esomeprazole was established,and it as a pre-audit rule to apply to the review of inpatient medical orders of esomeprazole was set.The medical records of patients who used esomeprazole sodium for injection before(January to May 2022,140 cases)and after(August to December 2022,140 cases)the pre-audit of prescriptions in our hospital were retrospectively reviewed.The rationality of the pre-audit was evaluated by the attribute hierarchical model(AHM)method and the intervention effect was analyzed.Results The total number of orders submitted to pharmacists for review by the system after pre-prescription review was 86,accounting for 7.96%;there were 58 cases(5.37%)of unreasonable medical orders returned by pharmacists;there were 49 cases(84.48%)of doctor's modified medical orders,and the success rate of prescription intervention gradually increased.After pre-prescription review,the proportion of 9 indicators such as indications,usage and dosage that met the evaluation criteria increased significantly.After pre-prescription review,the average score of each medical record score(MRS)was(96.14±1.46)points,which was significantly higher than(88.74±5.26)points before pre-prescription review(P<0.05).The number of MRS ≥ 90 points increased from 97 cases(69.29%)to 130 cases(92.86%).Conclusion Based on the DUE standard,the audit rules are set up for the pre-audit of esomeprazole in inpatient medical orders,which helps to standardize the prescription behavior of doctors,improve the quality of medical orders,and effectively promote the rational use of esomeprazole in clinical practice.
