Influence of 4+7 centralized procurement policy on the use of clinical antibiotics in our hospital
Objective To explore the influence of 4 + 7 centralized procurement policy(referred to as the collection policy)on the use of clinical antibiotics in our hospital.Methods The hospital information system was used to collect and analyze the number of varieties,sales amount,defined daily dose system(DDDs)and defined daily cost(DDC)of antibiotics,and to compare the clinical use of antibiotics in our hospital from January 2018 to November 2019(before the implementation of the collection policy)and from December 2019 to December 2022(after the implementation of the collection policy).Results The antibiotics used in the hospital included cefuroxime axetil tablets,azithromycin tablets,etc;after the implementation of the collection policy,the number of antibacterial drugs increased by 2.46%,the top three varieties of antibacterial drugs were cefuroxime axetil tablets,levofloxacin tablets and ciprofloxacin tablets,among them,the number of cefuroxime axetil tablets increased the most significantly,with an increase of 36.32%compared with that before the implementation of the collection policy.After the implementation of the collection policy,the sales amount of antibacterial drugs decreased by 84.68%compared with that before the implementation of the collection policy;the sales amount of levofloxacin tablets decreased the most,with a decrease of 93.46%.After the implementation of the collection policy,the top three drugs in DDDs values of antibiotics were azithromycin tablets,levofloxacin tablets and cefuroxime axetil tablets.After the implementation of the collection policy,the top three drugs in DDC values of antibiotics were levofloxacin sodium chloride injection,azithromycin for injection and ciprofloxacin tablets.Conclusion The 4 + 7 collection policy has effectively reduced the sales amount of some clinical antibiotics and increased the frequency of use.Clinical attention should be paid to the rational use of antibiotics and drug abuse.
