摘要
目的 研究高血压性脑出血(HICH)复发出血的危险因素,为疾病防治及预后评价提供参考.方法 选取2021年 1 月至 2022 年 6 月安康市中医医院收治的 588 例HICH患者为研究对象,根据纳排标准最终纳入570例患者.收集患者的临床资料,采用单因素及多因素Logistic回归分析探讨影响HICH复发出血的独立危险因素.结果 单因素分析结果显示,HICH复发患者的年龄、吸烟史、发病季节、中医体质、冠心病、糖尿病、脑梗死、出院血压、出血量及同型半胱氨酸(Hcy)水平与未复发患者比较,差异具有统计学意义(P＜0.05).多因素Logistic回归分析结果显示,年龄>65 岁、冠心病、糖尿病、脑梗死病史以及出院血压异常是影响HICH复发出血的独立危险因素(P=0.013、0.044、0.038、0.009、0.003).结论 高龄、冠心病、糖尿病、脑梗死病史以及出院血压异常是HICH患者复发出血的独立危险因素.
Abstract
Objective To study the risk factors for recurrent hemorrhage of hypertensive intracerebral hemorrhage(HICH),and to provide references for disease prevention and prognosis evaluation.Methods A total of 588 patients with HICH admitted in Ankang Hospital of Traditional Chinese Medicine from January 2021 to June 2022 were selected as the research objects,and 570 patients were finally included according to the inclusion and exclusion criteria.The clinical data of the patients were collected,and univariate and multivariate Logistic regression analysis were used to explore the independent risk factors affecting HICH recurrent hemorrhage.Results The results of univariate analysis showed that the age,smoking history,onset season,TCM constitution,coronary heart disease,diabetes,cerebral infarction,discharge blood pressure,bleeding volume and homocysteine(Hcy)of HICH recurrence patients were significantly different from those of non-recurrence patients(P<0.05).Multivariate Logistic regression analysis showed that age>65 years old,history of coronary heart disease,diabetes,cerebral infarction and abnormal blood pressure at discharge were independent risk factors for recurrent hemorrhage of HICH(P=0.013,0.044,0.038,0.009,0.003).Conclusion Advanced age,history of coronary heart disease,diabetes,cerebral infarction and abnormal blood pressure at discharge are independent risk factors for recurrent hemorrhage in HICH patients.
