Application value of fecal mSDC2 detection in colorectal neoplasms screening in physical examination population
Objective To analyze the screening results of methylated syndecan-2(mSDC2)detection in colorectal neoplasms(CN),and to evaluate its potential and value in CN screening in physical examination population.Methods The patients who received mSDC2 detection and completed colonoscopy in the health management center of Peking University Shenzhen Hospital from March 2021 to July 2022 were retrospectively analyzed.The mSDC2 detection was used as the primary screening method,and the colonoscopy and pathological results were used as the gold standard to analyze the screening results.Results A total of 323 patients received mSDC2 test and completed colonoscopy,there was no significant difference in mSDC2 test results between patients of different genders and age groups(P>0.05).The results of mSDC2 test were positive in 88 cases,gray area in 68 cases,and negative in 167 cases.Among the 323 subjects,there were 4 cases of colorectal cancer,29 cases of advanced adenoma(AA),63 cases of non-advanced adenoma(NAA),54 cases of non-adenomatous polyp(NAP)and 173 other cases(including 9 cases of normal intestine,163 cases of enteritis or/and diverticulum,and 1 case of melanosis);the relative sensitivity of mSDC2 to colorectal cancer was 100.00%,and the relative sensitivity to AA was 62.07%.The detection rates of colorectal cancer and AA in mSDC2 positive were 4.55%and 20.45%,respectively.There was statistically significant difference in the detection rates of colorectal cancer and AA among positive,gray area and negative(P<0.05);there was no significant difference in the detection rate of NAA and NAP among positive,gray area and negative(P>0.05).In CN,the detection rate of mSDC2 positive,gray area and negative was statistically significant(P<0.05);in all intestinal lesions,the detection rate of mSDC2 positive,gray area and negative was statistically significant(P<0.05).Conclusion As a preliminary screening method,the detection of fecal mSDC2 significantly improves the detection rate of lesions in colonoscopy,and can be used as a preferred strategy for early screening of CN in physical examination population.
