目的 探讨基于3D打印模型辅助后柱螺钉内固定用于髋臼骨折的可行性和安全性.方法 选取2020年1月至2021年6月本院收治的 80 例髋臼骨折患者为研究对象,依据入院编号奇偶性将其分为对照组和观察组,每组40例.对照组行腹直肌旁入路切开复位螺钉内固定术,观察组行基于 3D打印模型辅助后柱螺钉内固定.比较两组的治疗效果.结果 术后6个月,观察组的Fugl-Meyer运动功能评定量表(FMA)、Harris髋关节功能评分(HHS)、日常生活活动能力量表(ADL)评分显著高于对照组,差异具有统计学意义(P<0.05).观察组的并发症总发生率显著低于对照组,差异具有统计学意义(P<0.05).观察组的手术时长、住院时间及骨愈合时间显著短于对照组,术中失血量显著少于对照组,差异具有统计学意义(P<0.05).术后2d,观察组的CD4+、CD4+/CD8+、白细胞介素-10(IL-10)水平显著高于对照组,CD8+、白细胞介素-6(IL-6)水平显著低于对照组,差异具有统计学意义(P<0.05).观察组的远期恢复优良率显著高于对照组,差异具有统计学意义(P<0.05).结论 基于3D打印模型辅助后柱螺钉内固定可显著提升髋臼骨折治疗效果,促进患者髋臼功能恢复,具有重要的临床应用价值.
Objective To explore the feasibility and safety of 3D printed model-assisted posterior column screw internal fixation for acetabular fracture.Methods Eighty patients with acetabular fracture admitted in our hospital from January 2020 to June 2021 were selected as the study objects and divided into control group and observation group according to the parity of admission numbers,with 40 cases in each group.The control group underwent open reduction screw fixation via rectus abdominis side approach,and the observation group underwent 3D printed model-assisted posterior column screw internal fixation.The therapeutic effects of the two groups were compared.Results At 6 months after surgery,the Fugl-Meyer Assessment(FMA),Harris Hip Score(HHS)and Activities of Daily Living(ADL)scores in the observation group were significanty higher than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of complications of the observation group was significanty lower than that of the control group,and the difference was statistically significant(P<0.05).The operation duration,hospital stay and bone healing time of the observation group were significanty shorter than those of the control group,the intraoperative blood loss was significanty less than that of control group,and the differences were statistically significant(P<0.05).At 2 d after surgery,the CD4+,CD4+/CD8+and interleukin-10(IL-10)level in the observation group were significantly higher than those in the control group,while the CD8+and interleukin-6(IL-6)level were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The excellent and good rate of long-term recovery of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion 3D printed model-assisted posterior column screw internal fixation can significantly improve the treatment effect of acetabular fracture and promote the recovery of acetabular function of patients,which has important clinical application value.
