Effects of proximal femoral nail antirotation on bone metabolism and bone strength indexes in elderly patients with intertrochanteric femoral fracture
Objective To investigate the value of proximal femoral nail antirotation(PFNA)in the treatment of elderly patients with intertrochanteric femoral fracture.Methods Sixty elderly patients with intertrochanteric femoral fracture admitted in our hospital from January 2019 to July 2023 were selected as the study objects and divided into control group(n=30)and observation group(n=30)according to random number table method.The control group was treated with dynamic hip screw,and the observation group was treated with PFNA.The therapeutic effects of the two groups were compared.Results The operative time and mean healing time of fracture in the observation group were shorter than those in the control group,and the amount of intraoperative blood loss was lower than that in the control group(P<0.05).After treatment,the Visual Analogue Scale(VAS)score of the observation group was lower than that of the control group,and the Harris Hip Score(HHS)score was higher than that of the control group(P<0.05).After treatment,the levels of osteocalcin(BGP)and N-terminal propeptide of typeⅠprocollagen(PⅠNP)in the observation group were higher than those in the control group,and the level of typeⅠcollagen C-terminal peptide(CTX-Ⅰ)was lower than that in the control group(P<0.05).After treatment,the levels of interleukin-6(IL-6),interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α)in the observation group were lower than those in the control group(P<0.05).After treatment,the buckling ratio(BR)of the observation group was lower than that of the control group,and the cross-sectional moment of inertia(CSMI)and bone mineral density(BMD)were higher than those of the control group(P<0.05).The total incidence of complications of the observation group was lower than that of the control group(P<0.05).Conclusion PFNA has a significant effect in the treatment of elderly patients with intertrochanteric femoral fracture,which can improce the bone metabolism and bone strength indexes.
