Clinical effect of levosimendan in the treatment of elderly patients with acute anterior myocardial infarction undergoing PPCI combined with heart failure during perioperative period
Objective To observe the clinical effect of levosimendan in the treatment of elderly patients with acute anterior myocardial infarction undergoing primary percutaneous coronary intervention(PPCI)combined with heart failure during perioperative period.Methods From January 2021 to January 2022,100 elderly patients with acute anterior myocardial infarction who underwent PPCI and complicated with left heart failure(Killip grade Ⅱ-Ⅲ)within 72 h of onset admitted in the Linyi Central Hospital were selected as the research objects.The patients were randomly divided into control group and observation group,with 50 cases in each group.The control group was given routine treatment,and the observation group was given levosimendan on the basis of routine treatment.The level of N-terminal pro-B-type natriuretic peptide(NT-proBNP)and cardiac function indexes[left ventricular end diastolic diameter(LVEDd),left ventricular end systolic diameter(LVEDs)and left ventricular ejection fraction(LVEF)]were compared between the two groups before treatment and at different time after treatment;the length of hospitalization in the two groups was recorded,and the rehospitalization rate and mortality rate were recorded after 1 year of follow-up.Results At 24 h,72 h and 7 d after treatment,the level of NT-proBNP in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).After treatment,LVEDd and LVEDs in the observation group were significantly lower than those in the control group,LVEF was significantly higher than that in the control group,and the differences were statistically significant(P<0.05).The length of hospitalization in the observation group was shorter than that in the control group,and the 1-year rehospitalization rate was significantly lower than that in the control group,and the differences were statistically significant(P<0.05);there was no significant difference in mortality between the two groups(P>0.05).Conclusion Early application of levosimendan can better improve the cardiac function of elderly patients with acute anterior myocardial infarction undergoing PPCI combined with heart failure during perioperative period,shorten the length of hospitalization,and has good short-term and long-term efficacy.
