柴桂温胆汤加减治疗肝郁痰阻型失眠的效果及对睡眠质量、过度觉醒状态的影响

摘要
目的 探讨柴桂温胆汤加减治疗肝郁痰阻型失眠的效果及对睡眠质量、过度觉醒的影响.方法 选择2019年1月至2023年1月收治的200例肝郁痰阻型失眠患者为研究对象,根据随机数字表法将其分为对照组(100例,右佐匹克隆治疗)和观察组(100例,在对照组基础上联合柴桂温胆汤加减治疗).比较两组的治疗效果.结果 观察组的治疗总有效率高于对照组(P＜0.05).治疗后,观察组的匹兹堡睡眠质量指数(PSQI)、过度觉醒量表(HAS)评分低于对照组(P＜0.05).治疗后,观察组的褪黑素(MT)、多巴胺(DA)水平高于对照组(P＜0.05).两组的不良反应总发生率比较,差异无统计学意义(P＞0.05).结论 柴桂温胆汤加减治疗肝郁痰阻型失眠患者的效果满意,可提高睡眠质量,改善过度觉醒状态,促进MT、DA分泌,且不会增加不良反应发生风险.
Abstract
Objective To explore the effect of modified Chaigui Wendan decoction on insomnia of liver stagnation phlegm type and its influences on sleep quality and hyperarousal state.Methods A total of 200 patients with insomnia of liver stagnation phlegm type admitted from January 2019 to January 2023 were selected as the research objects.According to the random number table method,the patients were divided into control group(100 cases,treated with eszopiclone)and observation group(100 cases,treated with modified Chaigui Wendan decoction on the basis of the control group).The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P＜0.05).After treatment,the scores of Pittsburgh Sleep Quality Index(PSQI)and Hyperarousal Scale(HAS)in the observation group were lower than those in the control group(P＜0.05).After treatment,the levels of melatonin(MT)and dopamine(DA)in the observation group were higher than those in the control group(P＜0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P＞0.05).Conclusion The modified Chaigui Wendan decoction in the treatment of insomnia patients with liver stagnation phlegm type has a satisfactory effect.It can enhance sleep quality,improve hyperarousal state,promote the secretion of MT and DA,and will not increase the risk of adverse reactions.
