Application value of Xpert MTB/RIF detection in bronchoalveolar lavage fluid for early diagnosis of tuberculosis
Objective To analyze the application value of Xpert MTB/RIF detection in bronchoalveolar lavage fluid for early diagnosis of tuberculosis. Methods A total of 96 patients with suspected tuberculosis admitted in our hospital from March 2022 to March 2023 were selected as the research objects.Sputum, bronchoalveolar lavage fluid and serous cavity effusion specimens were collected from all objects after enrollment, and then Xpert MTB/RIF detection was performed respectively. The diagnostic efficacy of Xpert MTB/RIF detection in three specimens for tuberculosis was compared with chest X-ray of the lungs and immune examination(tuberculin skin test or interferon-gamma release test)as the gold standard. Results Among 96 patients with suspected tuberculosis,89 cases of tuberculosis and 7 cases of non-tuberculosis were detected by gold standard. There were no significant differences in the diagnostic accuracy, sensitivity, specificity, positive predictive value and negative predictive value of Xpert MTB/RIF detection in sputum and serous cavity effusion specimens for tuberculosis(P>0.05);the diagnostic accuracy, sensitivity, specificity, positive predictive value and negative predictive value of Xpert MTB/RIF detection of bronchoalveolar lavage fluid specimen for tuberculosis were significantly higher than those of sputum and serous cavity effusion specimens, and the differences were statistically significant(P<0.05).Conclusion In the early diagnosis of tuberculosis, the Xpert MTB/RIF detection in bronchoalveolar lavage fluid can achieve higher diagnostic efficiency.
