摘要
目的 探究达格列净片联合雷公藤多苷片治疗早期糖尿病肾病(DKD)的效果及对肾血流状态的影响.方法 选择 2020 年 1 月至 2022 年 11 月我院收治的 100 例早期DKD患者为研究对象,以随机数字表法将其分为对照组、观察组,每组50 例.对照组接受常规对症治疗+雷公藤多苷片,观察组在对照组治疗方案基础上加用达格列净片.比较两组的治疗效果.结果 观察组的治疗总有效率高于对照组(P＜0.05);两组的不良反应总发生率无明显差异(P＞0.05).治疗后,两组的舒张期最低血流速度(Vdmin)、收缩期最大血流速度(Vsmax)高于治疗前,搏动指数(PI)、阻力指数(RI)低于治疗前,且观察组优于对照组(P＜0.05).治疗后,两组的血尿素氮(BUN)、血肌酐(Scr)、胱抑素C(CysC)水平均低于治疗前,血清白蛋白(Alb)水平高于治疗前,且观察组优于对照组(P＜0.05).结论 达格列净片联合雷公藤多苷片治疗DKD效果显著,可改善肾血流状态,促进肾功能恢复,值得临床推广与应用.
Abstract
Objective To explore the effect of dapagliflozin tablets combined with tripterygium wilfordii polyglycoside tablets on early diabetic kidney disease(DKD)and its influence on renal blood flow status.Methods A total of 100 patients with early DKD admitted in our hospital from January 2020 to November 2022 were selected as the research objects,and the patients were divided into control group and observation group by random number table method,with 50 cases in each group.The control group received conventional symptomatic treatment+tripterygium wilfordii polyglycoside tablets,and the observation group was treated with dapagliflozin tablets on the basis of the treatment plan of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05);there was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).After treatment,the minimum diastolic blood flow velocity(Vdmin)and maximum systolic blood flow velocity(Vsmax)of both groups were higher than those before treatment,while the pulsatility index(PI)and resistance index(RI)were lower than those before treatment,and those in the observation group were better than the control group(P<0.05).After treatment,the levels of blood urea nitrogen(BUN),serum creatinine(Scr),and cystatin C(CysC)in both groups were lower than those before treatment,while the level of serum albumin(Alb)was higher than that before treatment,and those in the observation group were better than the control group(P<0.05).Conclusion The dapagliflozin tablets combined with tripterygium wilfordii polyglycoside tablets has a significant effect on the treatment of DKD,it can improve renal blood flow status and promote renal function recovery,which is worthy of promotion and application in clinic.
