Effects of laparoscopic cholecystectomy via posterior Calot's triangle anatomical approach on immune function and stress response in patients with chronic cholecystitis complicated with gallstones
Objective To study the effects of laparoscopic cholecystectomy via posterior Calot's triangle anatomical approach on immune function and stress response in patients with chronic cholecystitis complicated with gallstones.Methods A total of 180 patients with chronic cholecystitis and gallstones admitted in our hospital from January 2020 to January 2023 were selected as the research objects and divided into control group(n=90)and observation group(n=90)according to the random number table method.Laparoscopic cholecystectomy was performed in both groups.The control group was treated with Calot's triangle anatomical approach,and the observation group was treated with posterior Calot's triangle anatomical approach.The perioperative indexes,oxidative stress factors levels,immune function indexes and postoperative complications were compared between the two groups.Results The operation time,hospitalization time and intestinal function recovery time of the observation group were shorter than those of the control group,the intraoperative blood loss was less than that of the control group,and the differences were statistically significant(P<0.05).After 24 h of treatment,the levels of malondialdehyde(MDA)and 3-nitrotyrosine(3-NT)in the observation group were lower than those in the control group,the levels of superoxide dismutase(SOD)and glutathione(GSH)were higher than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the CD3+,CD4+and CD4+/CD8+in the observation group were higher than those in the control group,the CD8+was lower than that in the control group,and the differences were statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was 2.22%,which was lower than 10.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic cholecystectomy via posterior Calot's triangle anatomical approach is effective in the treatment of patients with chronic cholecystitis and gallstones.It can shorten the operation time,postoperative intestinal function recovery time and hospitalization time,reduce intraoperative blood loss,reduce oxidative stress response,improve immune function,and decline the risk of postoperative complications.
