Effects of posterior reduction and internal fixation with bone graft and vertebral pedicle screw in injured vertebra on the injured vertebral function and adverse events in patients with thoracolumbar fractures
Objective To explore the effects of posterior reduction and internal fixation with bone graft and vertebral pedicle screw in injured vertebra on the injured vertebral function and adverse events in patients with thoracolumbar fractures.Methods A total of 84 patients with thoracolumbar fractures who underwent internal fixation in our hospital from June 2020 to June 2022 were selected as the research objects.According to different surgical methods,the patients were divided into study group(42 cases,posterior reduction and internal fixation with bone graft and vertebral pedicle screw in injured vertebra)and control group(42 cases,posterior reduction and internal fixation across injured vertebra).The therapeutic effect,vertebral anatomical structure,recovery of injured vertebral function and incidence of adverse events were compared between the two groups.Results The excellent and good rate of treatment in the study group was higher than that in the control group(P<0.05).At 1,3 and 6 months after operation,the ratio of height of anterior edge of injured vertebra of the two groups increased,and the sagittal Cobb angle decreased,and those in the study group were better than the control group(P<0.05).At 1,3 and 6 months after operation,the Japanese Orthopaedic Association(JOA)score of the study group was higher than that of the control group,and the Oswestry Disability Index(ODI)score was lower than that of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse events between the two groups(P>0.05).Conclusion Posterior reduction and internal fixation with bone graft and vertebral pedicle screw in injured vertebra can improve the vertebral anatomical structure and lumbar function of patients with thoracolumbar fractures.It has definite curative effect and high safety,and has clinical application value.
