Application value of MCHC,MCV,MCH and RDW in the differential diagnosis of iron deficiency anemia and thalassemia
Objective To explore the application value of mean corpuscular hemoglobin concentration(MCHC),mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH)and red blood cell volume distribution width(RDW)in the differential diagnosis of iron deficiency anemia and thalassemia.Methods A total of 88 patients with iron deficiency anemia who were examined in our hospital from January 2021 to December 2022 were selected as iron deficiency anemia group,and 88 patients with thalassemia were selected as thalassemia group.In addition,88 healthy subjects in our hospital during the same period were included in control group.All three groups of research objects underwent blood routine tests,and the diagnostic results of MCHC,MCV,MCH and RDW for iron deficiency anemia and thalassemia were compared,and the diagnostic efficacy of single or combined test of each indexes for these two diseases was analyzed.Results MCHC and MCV in the thalassemia group were higher than those in the iron deficiency anemia group,and RDW was lower than that in the iron deficiency anemia group(P<0.001);MCHC,MCV and MCH in the iron deficiency anemia group were lower than those in the control group,and RDW was higher than that in the control group(P<0.001);MCV and MCH in the thalassemia group were significantly lower than those in the control group(P<0.001).The sensitivity and negative predictive value of MCHC+MCV+MCH+RDW test for the diagnosis of iron deficiency anemia were higher than those of MCHC,MCV and MCH tests,and the diagnostic coincidence rate was higher than that of MCH test(P<0.05).The sensitivity,negative predictive value and diagnostic coincidence rate of MCHC+MCV+MCH+RDW tests in the diagnosis of thalassemia were higher than those of MCV and MCH tests(P<0.05).Conclusion MCHC,MCV,MCH and RDW in anemia patients are significantly different from those in healthy people.The combined test of the four indexes can provide a reference for the differential diagnosis of iron deficiency anemia and thalassemia,and can improve the diagnostic efficacy of these two anemia diseases.
