Analysis of influencing factors of piperacillin-tazobactam related neutropenia and construction of risk model
Objective To analyze the influencing factors of piperacillin-tazobactam(PTZ)related neutropenia,and to construct a nomogram and risk prediction model.Methods The clinical data of 264 patients with pulmonary infection treated with PTZ in People's Hospital of Luchuan from January 2019 to June 2022 were retrospectively analyzed.Multivariate Logistic regression analysis,Lasso regression analysis and random forest analysis were used to explore the influencing factors of PTZ related neutropenia,and a nomogram model was constructed to identify the risk factors of neutropenia.Results The results of multivariate Logistic regression analysis,Lasso regression analysis and random forest analysis showed that male,advanced age,high body mass index(BMI),combined use of antibiotics,chronic obstructive pulmonary disease(COPD),high C-reactive protein(CRP),high alanine transaminase(ALT)and low creatinine clear rate(CCR)were risk factors for neutropenia(P<0.05).The C-index of constructed decision curve was 0.719,suggesting that the correction curve was close to the ideal curve.Hosmer-Lemeshow goodness of fit test showed that there was no difference between the predicted value and the measured value(P>0.05).Conclusion Male,advanced age,high BMI,combined use of antibiotics,COPD,high CRP and low CCR were the influencing factors of neutropenia.
