Effect of high-dose vitamin D combined with double C regimen in the treatment of gestational diabetes mellitus and its influences on islet function and pregnancy outcome

目的 分析高剂量维生素D结合双C方案治疗妊娠期糖尿病的效果及对胰岛功能、妊娠结局的影响.方法 选取2021年3月至2023年3月诊治的100例妊娠期糖尿病患者为研究对象,将其随机分为对照组和观察组,各50例.对照组采用双C方案治疗,观察组在对照组基础上加高剂量维生素D治疗.比较两组的治疗效果.结果 治疗后,观察组的空腹血糖(FPG)、餐后2 h血糖(2 h PG)水平及糖化血红蛋白(HbA1c)低于对照组,差异具有统计学意义(P＜0.05).治疗后,观察组的胰岛β细胞功能指数(HOMA-β)、胰岛素抵抗指数(HOMA-IR)、果糖胺(FMN)及空腹C肽(FCP)水平低于对照组,胰岛素敏感指数(ISI)高于对照组,差异具有统计学意义(P＜0.05).观察组的不良妊娠结局总发生率低于对照组,新生儿Apgar评分高于对照组,差异具有统计学意义(P＜0.05).结论 高剂量维生素D结合双C方案治疗妊娠期糖尿病的效果显著,能有效调节胰岛功能,改善妊娠结局,值得推广.
Objective To analyze the effect of high-dose vitamin D combined with double C regimen in the treatment of gestational diabetes mellitus and its influences on islet function and pregnancy outcome.Methods A total of 100 patients with gestational diabetes mellitus diagnosed and treated from March 2021 to March 2023 were selected as the research objects,and the patients were randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with double C regimen,and the observation group was treated with high-dose vitamin D on the basis of the control group.The therapeutic effects of the two groups were compared.Results After treatment,the levels of fasting plasma glucose(FPG),2 hours postprandial blood glucose(2 h PG)and glycosylated hemoglobin(HbA1c)in the observation group were lower than those in the control group,and the differences were statistically significant(P＜0.05).After treatment,the homeostasis model assessment-β(HOMA-β),homeostasis model assessment-insulin resistance index(HOMA-IR),fractosamine(FMN)and fasting C-peptide(FCP)levels in the observation group were lower than those in the control group,insulin sensitivity index(ISI)was higher than that in the control group,and the differences were statistically significant(P＜0.05)The total incidence of adverse pregnancy outcomes in the observation group was lower than that in the control group,the neonatal Apgar score was higher than that in the control group,and the differences were statistically significant(P＜0.05).Conclusion The high-dose vitamin D combined with double C regimen in the treatment of gestational diabetes mellitus has a significant effect,it can effectively regulate islet function and improve pregnancy outcome,which is worth promoting.
