Objective To explore the optimal dose selection of 0.5%isobaric levobupivacaine for low-plane spinal anesthesia in elderly patients.Methods A total of 100 elderly patients undergoing elective surgical treatment were selected and divided into group B(15 mg),group D(12.5 mg),group E(10 mg),group H(7.5 mg)and group I(5 mg)according to the different doses of 0.5%levobupivacaine injected into subarachnoid space,with 20 cases in each group.The anesthetic effect,hemodynamic indexes and adverse reactions of the five groups were compared.Results The onset time of sensory and motor block in the group H were longer than those in the group B,group D and group E,and those in the group I were longer than the group H(P<0.05);the duration of sensory and motor block in the group H were shorter than those in the group B,group D and group E,and those in the group I were shorter than the group H(P<0.05);the highest block level in the group H and group I was lower than that in the group B and group D(P<0.05);the proportion of anesthesia block plane higher than T10 in the group H was lower than that in the group B and group D,and that in the group I was lower than the group B,group D and group E(P<0.05);the anesthesia plane fixation time in the group H was shorter than that in the group B and group D,and that in the group I was shorter than the group E and group H(P<0.05);the Modified Bromage Score(MBS)score of the group H was lower than that of the group B,group D and E group,and that of the group I was lower than the group H(P<0.05).At 3 min after anesthesia(T2)to 15 min after anesthesia(T5),mean arterial pressure(MAP)and heart rate(HR)in the group B,group D,group E and group H were lower than those before anesthesia(T0)(P<0.05);at T2-T5,MAP and HR in the group B and group D were lower than those in the group I(P<0.05).The incidences of nausea and vomiting,chills and dyspnea in the group B and group D were higher than those in the group H and group I(P<0.05);the incidence of urinary retention in the group B was higher than that in the group H and group I(P<0.05).Conclusion 10 mg 0.5%levobupivacaine on elderly patients undergoing spinal anesthesia has satisfactory effect,with stable hemodynamics and less adverse reactions.
