Retrospective analysis of blood preparation strategies for pregnant women with unexpected antibody positive
Objective To analyze the blood preparation of pregnant women with unexpected antibody positive and explore the blood preparation strategy,so as to improve the safety and timeliness of obstetric blood use.Methods From December 2021 to December 2022,5 682 pregnant women in the third trimester of pregnancy who were tested for unexpected antibody in Northwest Women's and Children's Hospital were selected as the research objects.The antibody positive rate,blind matching and special matching rate,unexpected antibody distribution and clinical blood use of pregnant women with different parity were analyzed.Results One hundred and twenty-three cases of unexpected antibody positive were detected in 5 682 pregnant women;among the pregnant women with unexpected antibody positive,the positive rates of the first,second and third births were 1.55%,3.03%and 8.06%,respectively,there was a statistically significant difference in the positive rate of antibody screening among different parity(P<0.05).There was a statistically significant difference in the coincidence rate between blind blood matching and special blood matching(P<0.05).A total of 8 antibodies were detected in 34 cases of unexpected antibody,with a total of 39 cases,of which anti-E accounted for the highest proportion(53.85%).Among the 123 pregnant women with unexpected antibody positive,9 cases used blood during hospitalization,including 5 cases of blind blood matching and 6 cases of special blood matching,a total of 34 U of blood was prepared and 25 U of blood was used.Conclusion For pregnant women with unexpected antibody positive,the strategy of combining blind matching and special matching should be flexibly adopted in combination with the actual situation,and blood should be prepared in advance to ensure the safety of maternal blood use.
