Effect of CVVH+HP blood purification mode on inflammatory factors and matrix metalloproteinase levels in patients with severe sepsis
Objective To explore the effect of continuous venovenous hernofiltration(CVVH)+hemoperfusion(HP)blood purification mode on the levels of inflammatory factors and matrix metalloproteinase in patients with severe sepsis.Methods A total of 68 patients with severe sepsis admitted in our hospital from January 2019 to December 2022 were selected as the research objects and divided into control group and observation group by random number table method,with 34 cases in each group.Both groups were given routine symptomatic treatment.On this basis,the control group was given HP blood purification mode,and the observation group was treated with CVVH on the basis of the control group.The therapeutic effects of the two groups were compared.Results Before treatment,there were no significant differences in the levels of interleukin-1(IL-1),interleukin-8(IL-8),high-sensitivity C-reactive protein(hs-CRP)and interleukin-1β(IL-1β)between the two groups(P>0.05);after treatment,the levels of IL-1,IL-8,hs-CRP and IL-1β in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of matrix metalloproteinase-9(MMP-9)and matrix metalloproteinase-2(MMP-2)between the two groups(P>0.05);after treatment,the levels of MMP-9 and MMP-2 in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse events between the two groups(P>0.05).Before treatment,there were no significant differences in Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)and modified Marshall score between the two groups(P>0.05);after treatment,the APACHEⅡ and modified Marshall scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The CVVH+HP blood purification mode in the treatment of severe sepsis can improve the expression of inflammatory factors and matrix metalloproteinases,relieve organ failure,improve the outcome of patients,and do not increase the incidence of adverse events,which is worthy of promotion.
