CVVH+HP血液净化模式对严重脓毒血症患者炎症因子及基质金属蛋白酶水平的影响

目的 探究连续性静脉-静脉血液滤过(CVVH)+血液灌流(HP)血液净化模式对严重脓毒血症患者炎症因子及基质金属蛋白酶水平的影响。方法 选择 2019 年 1 月至 2022 年 12 月我院收治的 68 例严重脓毒血症患者为研究对象,以随机数字表法将其分为对照组、观察组,每组 34例。两组均给予常规对症治疗,在此基础上,对照组给予HP血液净化模式,观察组在对照组基础上加施CVVH治疗。比较两组的治疗效果。结果 治疗前,两组的白细胞介素-1(IL-1)、白细胞介素-8(IL-8)、超敏C-反应蛋白(hs-CRP)及白细胞介素-1β(IL-1β)水平比较,差异无统计学意义(P>0。05);治疗后,观察组的IL-1、IL-8、hs-CRP及IL-1β水平低于对照组,差异具有统计学意义(P<0。05)。治疗前,两组的基质金属蛋白酶-9(MMP-9)、基质金属蛋白酶-2(MMP-2)水平比较,差异无统计学意义(P>0。05);治疗后,观察组的MMP-9、MMP-2 水平低于对照组,差异具有统计学意义(P<0。05)。两组的不良事件总发生率比较,差异无统计学意义(P>0。05)。治疗前,两组的急性生理及慢性健康状况评分Ⅱ(APACHEⅡ)、改良Marshall评分比较,差异无统计学意义(P>0。05);治疗后,观察组的APACHEⅡ、改良Marshall评分低于对照组,差异具有统计学意义(P<0。05)。结论 CVVH+HP血液净化模式治疗严重脓毒血症可改善炎症因子及基质金属蛋白酶表达,缓解器官衰竭,改善患者结局,且不增加不良事件发生率,值得推广。
Effect of CVVH+HP blood purification mode on inflammatory factors and matrix metalloproteinase levels in patients with severe sepsis
Objective To explore the effect of continuous venovenous hernofiltration(CVVH)+hemoperfusion(HP)blood purification mode on the levels of inflammatory factors and matrix metalloproteinase in patients with severe sepsis.Methods A total of 68 patients with severe sepsis admitted in our hospital from January 2019 to December 2022 were selected as the research objects and divided into control group and observation group by random number table method,with 34 cases in each group.Both groups were given routine symptomatic treatment.On this basis,the control group was given HP blood purification mode,and the observation group was treated with CVVH on the basis of the control group.The therapeutic effects of the two groups were compared.Results Before treatment,there were no significant differences in the levels of interleukin-1(IL-1),interleukin-8(IL-8),high-sensitivity C-reactive protein(hs-CRP)and interleukin-1β(IL-1β)between the two groups(P>0.05);after treatment,the levels of IL-1,IL-8,hs-CRP and IL-1β in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of matrix metalloproteinase-9(MMP-9)and matrix metalloproteinase-2(MMP-2)between the two groups(P>0.05);after treatment,the levels of MMP-9 and MMP-2 in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse events between the two groups(P>0.05).Before treatment,there were no significant differences in Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)and modified Marshall score between the two groups(P>0.05);after treatment,the APACHEⅡ and modified Marshall scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The CVVH+HP blood purification mode in the treatment of severe sepsis can improve the expression of inflammatory factors and matrix metalloproteinases,relieve organ failure,improve the outcome of patients,and do not increase the incidence of adverse events,which is worthy of promotion.
