目的 探讨吻合器痔上黏膜环切术联合闭合性痔切除术治疗环状混合痔的效果及对疼痛程度的影响。方法 选取2022 年2月至2023 年 2月我科收治的 98 例环状混合痔患者作为研究对象，随机将其分为对照组(开放性痔切除术)和观察组(吻合器痔上黏膜环切术联合闭合性痔切除术)，各 49 例。比较两组的手术疗效。结果 观察组的手术时间、住院时间、切口愈合时间均短于对照组，术中出血量少于对照组，视觉模拟评分法(VAS)评分低于对照组(P＜0。05)。术后 1d，观察组的P物质(SP)、前列腺素E2(PGE2)及缓激肽(BK)水平低于对照组(P＜0。05)。术后1d，观察组的血管内皮生长因子(VEGF)、血小板源性生长因子(PDGF)及碱性成纤维细胞生长因子(bFGF)水平高于对照组(P＜0。05)。观察组的并发症总发生率低于对照组(P＜0。05)。结论 吻合器痔上黏膜环切术联合闭合性痔切除术治疗环状混合痔可缩短手术时间，减少术中出血，加快术后恢复，减轻术后疼痛，促进创面恢复，也能降低并发症发生率，值得推广。
Effect of procedure for prolapse and hemorrhoids combined with closed hemorrhoidectomy in the treatment of circular mixed hemorrhoids and its influence on pain degree
Objective To explore the effect of procedure for prolapse and hemorrhoids combined with closed hemorrhoidectomy in the treatment of circular mixed hemorrhoids and its influence on pain degree.Methods A total of 98 patients with circular mixed hemorrhoids admitted in our department from February 2022 to February 2023 were selected as the study objects and randomly divided into control group(open hemorrhoids resection)and observation group(procedure for prolapse and hemorrhoids combined with closed hemorrhoids resection),with 49 cases in each group.The surgical efficacy of the two groups was compared.Results The operation time,hospitalization time and incision healing time of the observation group were shorter than those of the control group,the intraoperative blood loss was less than that of the control group,and the Visual Analogue Scale(VAS)score was lower than that of the control group(P<0.05).One day after operation,the levels of substance P(SP),prostaglandin E2(PGE2)and bradykinin(BK)in the observation group were lower than those in the control group(P<0.05).One day after operation,the levels of vascular endothelial growth factor(VEGF),platelet-derived growth factor(PDGF)and basic fibroblast growth factor(bFGF)in the observation group were higher than those in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion The procedure for prolapse and hemorrhoids combined with closed hemorrhoidectomy in the treatment of circular mixed hemorrhoids can shorten the operation time,decrease intraoperative bleeding,accelerate postoperative recovery,alleviate postoperative pain,promote wound recovery,and also reduce the incidence of complications,which is worthy of promotion.
