Comparison of the diagnostic value of placental villus vascular parameters and umbilical artery blood flow parameters in fetal growth restriction
Objective To compare the diagnostic value of placental villus vascular parameters and umbilical artery blood flow parameters in fetal growth restriction(FGR).Methods A total of 92 pregnant women with FGR diagnosed in our hospital from March 2020 to March 2023 were collected as observation group, and 92 pregnant women with normal fetal development were collected as control group according to the ratio of 1:1.The umbilical artery blood flow parameters and placental villus vascular parameters of the two groups were measured and compared. The influencing factors of FGR and the diagnostic value of umbilical artery blood flow parameters and placental villus vascular parameters for FGR were analyzed. Results The peak systolic velocity/end diastolic velocity(S/D),resistance index(RI)and pulsatility index(PI)in the observation group were higher than those in the control group, and the vascularity index(VI),vascularization flow index(VFI)and flow index(FI)were lower than those in the control group, and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that S/D,RI and PI were risk factors for FGR,and VI,VFI and FI were protective factors(P<0.05).The results of receiver operating characteristic(ROC)curve showed that the diagnostic efficacy of combined detection of S/D,RI and PI was better than that of single detection of S/D,RI and PI(P<0.05);the diagnostic efficacy of combined detection of VI,VFI and FI was better than that of single detection of VI,VFI and FI(P<0.05);the combination of placental villus vascular parameters was superior to umbilical artery blood flow parameters in the diagnosis of FGR(P<0.05).Conclusion The abnormal umbilical artery blood flow parameters and placental villus vascular parameters are closely related to the occurrence of FGR.Compared with the combined detection of umbilical artery blood flow parameters, the combined detection of placental villus vascular parameters can more effectively determine the occurrence of FGR.
