Clinical value of 12-lead dynamic electrocardiogram combined with CT angiography in the diagnosis of asymptomatic myocardial ischemia
Objective To analyze the diagnostic value of 12-lead dynamic electrocardiogram combined with CT angiography in asymptomatic myocardial ischemia. Methods A total of 88 patients with suspected asymptomatic myocardial ischemia diagnosed and treated in our hospital from June 2020 to May 2022 were included as the study objects. All patients underwent 12-lead dynamic electrocardiogram,CT angiography and coronary angiography examination. The results of 12-lead dynamic electrocardiogram and CT angiography were compared between the positive and negative groups of coronary angiography;with diagnostic results of coronary angiography as the"gold standard",the diagnostic value of 12-lead dynamic electrocardiogram,CT angiography alone and combined examination for asymptomatic myocardial ischemia was compared;the consistency of 12-lead dynamic electrocardiogram,CT angiography alone and combined examination with coronary angiography were analyzed. Results A total of 58 cases were positive and 30 cases were negative detected by coronary angiography;the QT interval and QTc interval in the positive group were significantly longer than those in the negative group(P<0.05).The number of plaques, total plaque volume, total plaque load, degree of vascular stenosis and reconstruction index in the positive group were higher than those in the negative group(P<0.05).The accuracy of combined examination of 12-lead dynamic electrocardiogram and CT angiography in the diagnosis of asymptomatic myocardial ischemia was higher than that of single examination(P<0.05).The combined examination of 12-lead dynamic electrocardiogram and CT angiography had good consistency with coronary angiography(Kappa=0.806).Conclusion 12-lead dynamic electrocardiogram combined with CT angiography has a good effect in the diagnosis of asymptomatic myocardial ischemia, and has important application value.
