Analysis of influencing factors of fungal infection in hospitalized patients with pulmonary tuberculosis
Objective To analyze the situation of fungal infection in hospitalized patients with pulmonary tuberculosis and the characteristics of pathogenic bacteria,and to explore the influencing factors of fungal infection.Methods A total of 439 hospitalized patients with pulmonary tuberculosis who were treated in the Sixth People's Hospital of Zhengzhou from January 2021 to December 2022 were selected as the research objects.Sputum or alveolar lavage fluid samples were taken for fungal culture,and the general data of the subjects and the levels of total protein(TP)and albumin(ALB)were collected retrospectively.The patients were divided into fungal infection group and non-fungal infection group according to whether the patients were complicated with fungal infection.The general data of the two groups were compared and the related factors that may affect the fungal infection of hospitalized patients with pulmonary tuberculosis were analyzed;multivariate Logistic regression analysis was used to explore the independent influencing factors of fungal infection in hospitalized patients with pulmonary tuberculosis.Results There were 98 cases of fungal infection in 439 hospitalized patients with pulmonary tuberculosis,and the infection rate was 22.32%.Among the 126 strains of fungi isolated,Candida albicans infection was the main,accounting for 46.83%(59 strains).The proportions of age ≥55 years old,course of disease ≥5 years,combined with other lung diseases,combined with diabetes,broad-spectrum antibiotics application time ≥14 d,glucocorticoids application time ≥7 d,lesion range ≥3 lung fields and invasive operation in the fungal infection group were significantly higher than those in the non-infected fungal group,while the body mass index(BMI),TP and ALB levels were significantly lower than those in the non-infected fungal group(P<0.05).Multivariate Logistic regression analysis results showed that combined with diabetes,broad-spectrum antibiotics application time ≥14 d,glucocorticoid application time ≥7 d,lesion range ≥3 lung fields,TP and ALB were independent influencing factors of fungal infection in hospitalized patients with pulmonary tuberculosis(P<0.05).Conclusion The risk of fungal infection in hospitalized patients with pulmonary tuberculosis was high,mainly Candida albicans infection,combined with diabetes,broad-spectrum antibiotics application time,glucocorticoid application time,lesion range,TP and ALB were the influencing factors of fungal infection.
