目的 分析门冬胰岛素50对妊娠期糖尿病(GDM)患者餐后2 h血糖(2 h PG)、空腹血糖(FPG)水平的影响。方法 选取2020年5月至2023年1月收治的60例GDM患者为研究对象，根据治疗方案的不同将其分为对照组(盐酸二甲双胍缓释片)与研究组(门冬胰岛素50+盐酸二甲双胍缓释片)，各30例。比较两组的治疗效果。结果 治疗后，研究组的FPG、2 h PG、夜间血糖水平及糖化血红蛋白(HbA1c)低于对照组，差异具有统计学意义(P＜0。05)。治疗后，研究组的内脂素、瘦素、C反应蛋白(CRP)、同型半胱氨酸(Hcy)水平低于对照组，脂联素(ADPN)水平高于对照组，差异具有统计学意义(P＜0。05)。治疗后，研究组胎儿脐动脉、肾动脉的收缩期峰值流速/舒张末期流速(S/D)、阻力指数(RI)低于对照组，差异具有统计学意义(P＜0。05)。两组的不良妊娠结局总发生率比较，差异无统计学意义(P＞0。05)。结论 门冬胰岛素50治疗GDM可调节脂肪炎性因子水平，改善血流动力学，降低血糖，具有较高的应用价值。
Effects of insulin aspart 50 on 2 h PG and FPG levels in patients with gestational diabetes mellitus
Objective To analyze the effects of insulin aspart 50 on 2 h postprandial plasam glucose(2 h PG)and fasting plasma glucose(FPG)levels in patients with gestational diabetes mellitus(GDM).Methods A total of 60 patients with GDM admitted from May 2020 to January 2023 were selected as the research objects.According to the different treatment schemes,the patients were divided into control group(metformin hydrochloride sustained-release tablets)and study group(insulin aspart 50+metformin hydrochloride sustained-release tablets),with 30 cases in each group.The therapeutic effects of the two groups were compared.Results After treatment,the FPG,2 h PG,nocturnal blood glucose levels and glycosylated hemoglobin(HbA1c)in the study group were lower than those in the control group,and the differences were statistically significant(P＜0.05).After treatment,the levels of visfatin,leptin,C-reactive protein(CRP)and homocysteine(Hcy)in the study group were lower than those in the control group,and the level of adiponectin(ADPN)was higher than that in the control group,and the differences were statistically significant(P＜0.05).After treatment,the peak systolic velocity/end diastolic velocity(S/D)and resistent index(RI)of fetal umbilical artery and renal artery in the study group were lower than those in the control group,and the differences were statistically significant(P＜0.05).There was no significant difference in the total incidence of adverse pregnancy outcomes between the two groups(P＞0.05).Conclusion Insulin aspart 50 in the treatment of GDM can regulate the levels of fat inflammatory factors,improve hemodynamics and reduce blood glucose,which has high application value.
