Objective To explore the relationship between hepcidin expression and cardiac function in elderly CHF patients with iron deficiency,and the effects of different ways of iron supplementation on iron metabolism,cardiac function and myocardial remodeling,and explore its clinical significance.Methods Ninety elderly CHF patients with i-ron deficiency treated at the First Affiliated Hospital of Hunan Normal University(Hunan Provincial People's Hospital)from March 2021 to December 2022 were selected.They were divided into routine treatment group,routine combind with oral iron supplementation treatment group and routine combind with intravenous iron supplementation treatment group,30 cases each group was given conventional anti-heart failure drugs,routine combind with oral iron supplementation treat-ment group was given routine combind with oral polysaccharide iron complex capsule treatment,and routine combind with intravenous iron supplementation treatment group and intravenous injection of dextran iron treatment group.Before treat-ment and after 8 weeks of treatment,the serum precursor B-type brain natriuretic peptide(NT-proBNP),6-minute walk test(6MWT),NYHA grade,left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),left atrial diameter(LA),interventricular septum thickness(IVST),left ventricular posterior wall thickness(LVPWT),serum hepcidin(Hep),hemoglobin(Hb),serum ferritin(SF),transferrin saturation(TS)were measured.Results Before and after treatment,the patients in the conventional treatment group,the conventional+oral iron sup-plementation treatment group,and the conventional+intravenous iron supplementation treatment group had NT-proBNP,6MWT,LVEF,and NYHA grades related to cardiac function;Myocardial remodeling related indicators LA;There was a statistically significant difference in iron metabolism related indicators Hep,Hb,SF,TS(P<0.05),while there was no statistically significant difference in LVEDD,IVST,and LVPWT among the three groups before and after treatment(P>0.05).There was a positive correlation(P<0.05)between serum ferritin and the levels of NT-proBNP and NYHA grading before treatment;Negative correlation with 6MWT,LVEF,Hb,TS,SF(P<0.05).Conclusions For elderly CHF patients with iron deficiency,iron supplementation can significantly improve iron deficiency,heart function and atri-al remodeling,and intravenous supplementation is more beneficial than oral supplementation.The level of serum hepcidin is negatively correlated with cardiac function.
