Objective To explore the predictive value of serum miR-20b,miR-24 and miR-137 levels at ad-mission for the prognosis in elderly patients with acute ischemic stroke(AIS).Methods A total of 138 elderly patients with AIS admitted to Department of Neurology in Affiliated Hospital of Xuzhou Medical University were enrolled between January 2022 and January 2023.According to scores of modified Rankin scale(mRS)at admission,they were divided into good prognosis group(≤2 points,n=82)and poor prognosis group(>2 points,n=56).The levels of serum miR-20b,miR-24 and miR-137 were detected by real-time fluorescence quantitative RT-PCR,and their predictive value for poor prognosis was analyzed by receiver operating characteristic(ROC)curves.The differences in covariates between good prognosis group and poor prognosis group were balanced by 1∶1 propensity score matching(PSM).The independ-ent risk factors of poor prognosis in elderly AIS patients were analyzed by multivariate logistic regression analysis.Re-sults The levels of total cholesterol and low-density lipoprotein in poor prognosis group were higher than those in good prognosis group,while high-density lipoprotein was lower than that in good prognosis group(P<0.05).The level of ser-um miR-20b in poor prognosis group was higher than that in good prognosis group,while levels of miR-24 and miR-137 were lower than those in good prognosis group(P<0.05).ROC curves analysis showed that AUC,sensitivity and speci-ficity of serum miR-20b,miR-24,miR-137 and combined detection for predicting poor prognosis were(0.751,0.707,0.732,0.812),(81.20%,74.40%,76.80%,92.90%)and(79.30%,73.50%,73.30%,69.50%),respectively.Multivariate analysis showed that miR-20b>1.78,miR-24<0.94 and miR-137<0.93 were independent risk factors of poor prognosis in elderly patients with AIS(P<0.05).Conclusion Combined detection of serum miR-20b,miR-24 and miR-137 has certain predictive value for the prognosis of elderly patients with AIS after admission.
