目的 探究城市社区中老年居民三酰甘油-葡萄糖指数(TyG)及其衍生指数三酰甘油-葡萄糖-体重指数(TyG-BMI)、三酰甘油-葡萄糖-腰围指数(TyG-WC)、三酰甘油-葡萄糖-腰臀比指数(TyG-WHR)与胰岛素抵抗(IR)的相关性,评估各指数用于中老年居民IR筛查及其评估价值.方法 选取2018年6月至2023年8月合肥市某社区50岁及以上中老年人748例,收集一般资料及实验室数据,根据是否存在IR将受试者分为IR组及非IR组.采用Pearson相关分析、多元线性回归分析和多元logistic回归分析各指数与IR的关系.结果 胰岛素抵抗指数(HOMA-IR)水平与TyG、TyG-BMI、TyG-WC、TyG-WHR呈正相关(P<0.01),调整IR的协变量后,与最低三分位数相比,TyG、TyG-BMI、TyG-WC、TyG-WHR最高三分位数发生IR的OR值(95%CI)分别为 12.68(6.61~24.34)、9.35(5.10~17.14)、13.04(6.69~25.41)、7.56(4.17~13.68),预测 IR 的受试者工作特征曲线下面积(AUC)分别为0.781、0.774、0.797、0.765,其中,TyG-WC的AUC最大,TyG-WHR的灵敏度最高.结论 TyG及其衍生指数TyG-BMI、TyG-WC、TyG-WHR是评估社区中老年人群IR的简便、相对准确的替代指标.
Objective To investigate the correlation between triglycerides glucose-body mass index(TyG)and its derivative indices and insulin resistance(IR)in middle-aged and elderly residents in urban communities,and to as-sess whether TyG and its derivative indices can be used for IR screening in elderly residents in urban communities.Methods People aged 50 years and above in an urban community in Hefei from June 2018 to August 2023(n=748)were selected for the study,and the data collected included general information and laboratory data,subjects were divided into 2 groups according to whether they were diagnosed with IR or not.Pearson correlation analysis,multiple linear re-gression analysis,and multiple logistic regression were used to analyze the relationship between every index and IR.Results The level of HOMA-IR was correlated with TyG,TyG-BMI,TyG-WC,and TyG-WHR(P<0.01),and after adjusting for covariates of IR,compared with the lowest tertile,the OR of IR occurring in the highest tertile of TyG,TyG-BMI,TyG-WC,and TyG-WHR values(95%CI)were 12.68(6.61-24.34),9.35(5.10-17.14),13.04(6.69-25.41),and 7.56(4.17-13.68),and the AUC for predicting IR were 0.781,0.774,0.797,and 0.765,with the AUC for TyG-WC was the largest and the sensitivity of TyG-WHR was the highest.Conclusion The TyG index and its derivatives,TyG-BMI,TyG-WC,and TyG-WHR are simple,relatively accurate and clinically usable surrogate indices for evaluating IR in middle-aged and elderly populations in urban communities.
