Objective To investigate the correlation between triglyceride glucose index(TyG index)and the risk of ketosis-prone type 2 diabetes.Methods Eighty-two newly diagnosed T2DM patients admitted to department of endocrinology of the First Affiliated Hospital of the University of Science and Technology of China from January 2021 to March 2024 were selected.They were divided into ketosis-prone type 2 diabetes group(KPD group,n=41)and non-ke-tosis onset group(NKPD group,n=41).The medical history and anthropometric data,ketone body in serum and urine,glucose and lipid metabolism,pancreatic islet function and other data of the two groups were collected.TyG index and in-sulin resistance index were calculated and compared.Moreover,based on the median value of TyG index,all subjects were redivided into low TyG index group and high TyG index group,then the difference of the prevalence of diabetic ke-tosis between the two sub-groups was compared.Binary logistic analysis was used to analyze the independent risk factors of the onset of ketosis-prone type 2 diabetes.Receiver operating(ROC)curves determine predictors of development in ketosis-prone type 2 diabetes.Results The TyG index in KPD group was significantly higher than that of the NKPD group[9.46(9.04,9.70)vs 9.66(9.21,11.09),Z=-2.397,P<0.05].In addition,the incidence of diabetic keto-sis in high TyG index group was significantly higher than that in low TyG index group(65.0%vs 35.7%,x2=7.03,P<0.05).Binary logistic regression analysis showed that TyG index and HbA1e were independent risk factors for the tendency of ketosis in newly diagnosed T2DM patients.ROC curve analysis showed that the area under the curve of TyG index combined with HbA1c for predicting the tendency of diabetic ketosis was 0.747(95%CI:0.641-0.852,P<0.05).On occasion of the optimal cutoff value of TyG index was 9.903,the corresponding sensitivity was 0.439 and the specificity was 0.878.Conclusions TyG index may be an independent risk factor for the onset of ketosis-prone type 2 diabetes.TyG index combined with HbA1c detection has higher predictive value in the onset of ketosis-prone T2DM pa-tients.
