摘要
目的:分析宫颈上皮内瘤变(CIN)患者的宫颈锥切特征与术后病变残留及HPV转阴之间的相关因素.方法:采用双中心回顾性研究分析507例CIN患者的基本资料、HPV状态、宫颈锥切手术特征以及术后结果.结果:患者的平均年龄为41.69±9.35岁.手术方式以LEEP为主(86.59%);术后病变残留率为7.89%.多因素逻辑回归分析揭示年龄、绝经、术前组织活检分级、锥切深度、内外切缘、腺体受累和术前多重HPV感染与病变残留显著关联(P<0.05);而锥高、腺体受累、术前多重HPV及高危HPV感染与HPV转阴显著关联(P<0.05).结论:腺体受累和术前多重HPV感染等因素与CIN患者宫颈锥切术后病变残留及HPV转阴相关,为制定个性化治疗方案提供了参考.
Abstract
Objective:To analyze the factors related to cervical conization characteristics,postoperative lesion re-siduals,and HPV clearance in patients with Cervical Intraepithelial Neoplasia(CIN).Methods:A bicentric retrospec-tive study was conducted,analyzing the basic information,HPV status,cervical conization surgery characteristics,and postoperative outcomes of 507 CIN patients.Results:The mean age of the patients was 41.69±9.35 years,with LEEP be-ing the primary surgery(86.59%).The postoperative lesion residual rate was 7.89%.Multivariate logistic regression anal-ysis revealed significant associations between lesion residuals and factors such as age,menopausal,preoperative tissue biopsy,cone depth,margins,gland involvement,and preoperative multiple HPV infections(P<0.05).Similarly,cone height,gland involvement,preoperative multiple HPV and high-risk HPV were significantly related to HPV clearance(P<0.05).Conclusion:Factors like gland involvement and preoperative multiple HPV infections are associated with the residuals of lesions post-cervical conization surgery and HPV clearance in CIN patients,providing valuable insights in developing personalized treatment plans.
