Impact of Diabetes Mellitus on Perioperative Complications of Total Knee Arthro-plasty:A Retrospective Analysis Study Based on the NSQIP Database
Objective:To analyze the characteristics of perioperative complications in patients undergoing joint ar-throplasty with comorbid diabetes.Methods:Data were sourced from the American College of Surgeons' National Surgi-cal Quality Improvement Program(NSQIP)database.Patients undergoing total knee arthroplasty(TKA)were divided into diabetes and non-diabetes groups.Propensity Score Matching(PSM)was used to match patients between the two groups,and the incidence rates of perioperative complications were compared.Multivariate logistic regression analysis was conducted to identify complications independently associated with diabetes.Results:Patients in the diabetes group had higher prevalence rates of comorbidities such as hypertension,obesity,hypothyroidism,chronic pulmonary dis-ease,depression,chronic renal disease,gout,and chronic heart failure.The incidence rates of postoperative anemia,electrolyte disturbances,acute kidney injury,urinary tract infections,respiratory failure,and acute pneumonia were also higher in the diabetes group.Multivariate regression analysis indicated that diabetes is a risk factor for these periop-erative complications.Conclusion:Patients with diabetes have a higher risk of developing postoperative anemia,electro-lyte disturbances,acute kidney injury,urinary tract infections,and other complications during the perioperative period of joint arthroplasty.Targeted interventions are necessary to reduce the occurrence of these complications.
